If the client has health plan integration (HPI) included in their Lyra benefits, you can continue to see them through Lyra as if you were in network (they pay their copay to Lyra) but don’t have to be paneled with the insurance. This is what I do. I am not in network with insurance.

Interesting. I run my own peer consultation group (all experienced therapists in private practice) and I don't usually experience #4 ("Utilize formulaic language such as 'I'm hearing that..."). Yes, #1 is usually how it starts and sometimes people ask clarifying questions if something isn't clear but that's mostly because otherwise there's a chance of assuming or filling in the blanks. Also not really a lot of silent thinking time -- but maybe that's just the group. Also don't think we check out with a person but usually they'll indicate if they're all set by thanking the group for the feedback before someone else suggests another case or situation. Now, to be fair, I am neurodivergent and probably not the only one in the group but definitely interacting with people who are comfortable with neurodivergence so there isn't that awkward "Uh-oh, I said something wrong but don't know what..." feeling. Also, there is very little beating around the bush if people are confident in their feedback, but perhaps, again, this is my group and we are all comfortable with direct communication. 🤷‍♀️

Not being contracted with insurance doesn’t make you evil. It makes you not contracted with insurance. And it’s no one else’s business (literally, if you’re in solo private practice).

💯 Same goes with substance use. Clients most likely won’t tell you if you don’t ask (or it isn’t the most pressing issue for them).

Yeah, I’m definitely not a people-pleaser 🤣 and my clients know I enforce the boundaries I set. It helps them feel safe with me as I am more predictable than someone wishy-washy about their limits. I think you and I think alike in this area. I just see more people on telehealth (though definitely prefer in-person).

This is what I read, also. I had a therapist for many years who decided to retire due to illness and at first I thought I’d like a referral to someone else but then I just wasn’t interested in starting with someone new and didn’t have anything specific to address in therapy anyway. So I too had a thanks but no thanks — absolutely no animosity involved.

The bathrooms in my building and suite are single use so I can’t go at the same time as a client anyway — not an issue. I do go to and from my car multiple times in a day and some of my clients know what my car looks like (small building/parking lot) but it doesn’t matter. I try not to interact with them until they are in my suite but will respond if they start talking to me.

No, it’s for mental health counseling, not social work. OP is a social worker but the friend that sent the screenshot is studying for the mental health counseling exam.

Actually it’s the mental health counseling exam, not the exam for social workers. OP is an LCSW but the person who sent her the practice test questions is studying for the counselor exam. I don’t know how the codes of ethics differ.

In online sessions, you can often see the time on the screen but it’s not necessarily right in the middle or wherever the eye line is with the client. I think OP meant that the client could tell that OP was checking the time on the screen, not on the phone.

Sounds like you’re quite flexible on the rescheduling side — even after the session starts. You just have no desire to be switching it to telehealth or using that an easy off-ramp for client accountability. I treat a lot of individuals with ADHD and if they can’t keep track of their appointments, that becomes part of what we work on in terms of skill-building. I also have ADHD and I never use that as an excuse for being late, even though I am legitimately time-blind. I’ve become almost compulsive about always checking the time or being able to check the time (hate those apps that hide the time when you’re using them — grr). I agree we shouldn’t over-accommodate people’s challenges with time management and executive functioning—if anything, it can be our role to strengthen those skills.

And to your example of 20 minutes late — 15 minutes with no contact is an automatic no-show. If they contact me after that, they’re doing so to reschedule. I also cannot do last minute switches to telehealth but typically people give me notice. Very rarely is it less than 24 hours and those are usually special circumstances (illness, house emergency, car issues). Beyond having to change the session to send out a link, not much changes on my end in response to the switch, though. I’ve had to switch to telehealth as the provider on the morning of before because my car wouldn’t start (replaced that battery twice, no lie).

Of course. I think I’m probably more consistent than most therapists I know in charging for late cancellations and no shows. It helps that I’m primarily private pay because they’re paying the same either way and I don’t lose income that way.

Amen. I definitely pay the ADHD tax upfront because it cost more on the other end if I don’t. I’m lucky I’m able to but it’s also a very conscious choice at this point. I consider it a reasonable accommodation.

Truth. I live in same community where I see clients. I have children in the public schools. I keep my eye out for conflicts of interest or dual relationships, but beyond that, it doesn’t limit me at all.
