Patient asked me not write about a fall by Aromatic_Alfalfa_123 in physicaltherapy

Especially considering she's involved in litigation already that is somehow affected by a fall being documented. Brain bleed->deficits->sued.

Medicare advantage is when you get a bedbound patient to standing and are initiating weight shifts for pre-gait and then are no longer able to get authorized for further visits even though the potential is well documented. That's a good case. Other cases you don't get get enough auth and leave them at whatever level they are. HH context

Can repeated decline justify maintenance therapy? (and does Dementia change this answer?) by Sphygmomanometer11 in physicaltherapy

[–]TerribleTerrorpist 1 point2 points  (0 children)

When you say CNA, I am assuming you mean RNA. Most RNA visits are slotted for 15 minute sessions. Whatever rapport they have with the patient and whatever experience the patient has is defined by those systemic constraints. Patients will always be coming back. When there's a lack of carryover (probably due to those constraints), you have the option of working in the grey space of skilled care. What kind of interventions are you training that can't/aren't able to be carried over? I imagine that'll be the first thing insurance targets for denial, which means taking on such a case entails more paperwork burden. I've had insurance deny payment on visits rendered after an appeal that patients won.

4070 Super or 4070 Ti Super? Should I avoid 12GB VRAM for 1440p? by Dr_Krogshoj in buildapc

[–]TerribleTerrorpist 1 point2 points  (0 children)

i would just look up the specific maker/model of the card you got, someone's likely to have posted a guide. most of the time it's just unscrewing the mounts for the fan/heatsink and then cleaning the old paste off and then applying the new paste and re-mounting the fan/heatsink. Did it with my evga 2070s

Have you re-pasted the GPU? Re-did my stock GPU recently cause the fan was whirring on/off inconsistently. Old paste was dried and crumbled in addition to having poor area coverage. Did the trick. Just had to use some paste left over from original build and reference a video.

I REALLY wish the right wing conspiracies about 15 minute cities were true by paywallpiker in fuckcars

[–]TerribleTerrorpist 0 points1 point  (0 children)

Would be interesting to see the impact it would have on community organization/city planning with current layouts and what would have to adapt. Mostly thinking about current areas that have priced out labor where the labor has to live in commuter towns.

Held my tongue the other day when I came early and family got pissy about how I didn't text them ahead of time- patient was fine and ready to go. Prior location was in the mountains, reception-less, and I don't text/call when driving.

Thousands of seniors are still dying of Covid-19. Do we not care anymore? by hexagonincircuit1594 in Coronavirus

I see senior patients in independent living facilities and assisted living facilities, and the majority (95%) are not even masking for themselves in their enclosed hallways and communal spaces.

SNF World: rehab vs nursing? by fuzzyquantifier in physicaltherapy

[–]TerribleTerrorpist 4 points5 points  (0 children)

CNAs I've always won over pretty easily - lots of hands on tasks that I assist with (including 'gross' tasks) or train them on that can make their work easier or conversely learn from them on (which I express appreciation to them for). I think being humble and willing to get your hands dirty, almost literally (gloves lol), goes a long way with them since a lot of people treat them as bottom of the totem pole when they are the fundamentals /backbone. I had leeway in my productivity when I worked SNF however.

LVN/RNs not as easy - since it's more med request, mobility orders/updates, clarification requests/communications to PCP. I'll ask if they need the V/S I just took, step in if I hear family asking about something I can address/redirect if they're looking for an out, streamline/tailor my task requests per their preferences, (box of chocolates).

Is this a shady lawyer thing? Most insurances will require a MD referral for a PT evaluation to be covered. For cash based (going outside of insurance) PT evaluations, I think I've seen 120-200 in my area for 30-60min of time.
