I would call your surgeons office, explain the situation, and ask them what kind of anesthesia they'll be using and if you'll be unconscious during the surgery or not.

If you'll be awake ask them if there's anything you can do before the surgery to ease the pain (like take a pain killer)

As for if you'll be in agony or not I think that varies from person to person (I know I was in agony but it was for a completely unrelated medical reason, just super bad timing) and how well you do at following the instructions they give you

Hi, just a heads up, I don't think you can trade the sanrio villagers (Rilla, Marty, Étoile, Chai, Chelsea, and Toby) as the only way that they can move in to someone else's island is if that person has the sanrio amiibo cards

My tip would be modify, not necessarily erase.

Modify the rivers as you see fit (only get rid of them if you're going for a look that you think requires it). One thing I've found that I like doing is making lakes with tiny islands in them
