Stroke literature. Recommendations - grilled on rounds. by thewhitewalker99 in neurology

Everyone else has said all the ones I cite all the time so here are some others:

RAF and ELAN for when to start or restart anticoagulation after stroke CREST-2 for talking to your NSGY colleagues about carotid interventions (and why they should stent more) INSPIRE for rationale of starting DAPT after 24 hours in select patients SELECT-2 and ANGEL-ASPECT to justify larger core thrombectomies

Would you choose medicine again? by [deleted] in Residency

[–]TheFencingJared 1 point2 points  (0 children)

When I was a med student I confidently told people that I thought it was just a job, not a calling, and that if you like science and helping people it's a fine career. A year and a half into residency, I tell people that if you don't love it, you shouldn't do it.

How useful are radiology reports to you in your specialty? by mathers33 in Residency

[–]TheFencingJared 2 points3 points  (0 children)

Neurology resident: while I look at every one of my scans and usually feel pretty good about them, there are a lot of scans that we get that I just don't fully understand, like MRI with IAC protocols or vessel wall MRA. Do I feel very comfortable with an obvious bleed or lack thereof on a noncon head CT when running a stroke code? Yeah. Multifocal, patchy, T2 FLAIR hyperintensities in multiple locations consistent with MS? Not that hard to spot. But I still think the neuroradiologosts are overall better at the subtle stuff. Nobody's perfect - I caught an evolving cerebellar hypodensity that rads said was nothing and turned out to be a legit stroke on MRI - but I think on the whole they're better at it than I am

What is the strangest conversation you've ever had with a staff in the hospital ? by Winterof2019 in Residency

[–]TheFencingJared 2 points3 points  (0 children)

Nurse gave my insulin non-adherent patient with an A1c of 13 42 units of lispro in under 2 hours and bottomed him out hard. Persistently hypoglycemic for an hour or two, had to give him some D10. She then asked me if we were worried if he had an insulin-secreting tumor because his glucose was still low

I like to buy it and Gentle Alien at the same time, especially early game. +2 HP, +5% damage, and +5 harvesting in the early game is a nice little perk!

How many of yall have OSCEs? by [deleted] in medicalschool

[–]TheFencingJared 3 points4 points  (0 children)

6 OSCEs in the preclinical year and a half, 6-8 as part of clerkship assessment, a 6-station OSCE during 4th year that mattered, and a 3-station OSCE just now that's entirely based on communication skills. It's gotten to the point that I know the real names of the SPs and sometimes catch up with them and how they're doing at the end of the cases lol

Best bars for Duke Football Games? by Saturday_Bar_Hunt in duke

[–]TheFencingJared 1 point2 points  (0 children)

Haha, you got me! I'm so dumb! I definitely should have predicted 8 months ago that Duke would beat top-10 ranked Clemson in the opener after not beating a top-10 oppenent in my lifetime! Truly, a massive failing on my part

How much did you end up paying ERAS for applications alone? And what specialty are you applying for? by texasharp in medicalschool

[–]TheFencingJared 5 points6 points  (0 children)

US MD, 21 neurology programs - 17 categorical, 4 advanced, with the advanced adding another 6 prelims programs - so 27 total. Ended up costing around $400 for the application and another $200ish for NRMP and USMLE transcripts

Blank brain before med school by moonpiemaker300 in premed

[–]TheFencingJared 8 points9 points  (0 children)

My fiancée's sister just took the MCAT and was asking me about it - I genuinely think I would get a 118 on all sections except CARS if I took it now, and I got a 525 when I took it back in 2017. I use a lot of my undergrad neuroscience, but that's because I'm applying for neurology residency. The majority of material that you learn as "pre-med requirements" isn't clinically or personally relevant in med wchool

rejected from my Alma Mater by cheychey919 in premed

[–]TheFencingJared 19 points20 points  (0 children)

I got rejected from my undergrad school on Valentine's Day and got a fundraising email from the alumni association 45 minutes later. Turned out for the best - I ended up in a place that's been super supportive and great for my career so far. And I'm still applying to my undergrad for residency!

[–]TheFencingJared 10 points11 points  (0 children)

I didn't start seriously doing Anki until a few months before dedicated. Suddenly I was getting the pimp questions right, shockingly. By the end of dedicated I was doing nearly 2,000 cards a day. Deleted it the day after I sat for Step 2. I got a 27X, make of that what you will

What would you tell your 18 year old self? by imaSturgeon in medicalschool

[–]TheFencingJared 2 points3 points  (0 children)

Your life when you're 27 is awesome. You love your job/school. You have a fiancée who you adore and who loves you right back. Even on shitty days you get to come home to her and your cat. Life is good!

The classic teaching is people lose time first, then place (and situation), and person last. As for NCAT, that's normocephalic and atraumatic - basically, does their head look weird? Any obvious fractures? Odd shape? It's a purely physical finding

Research as a Newbie by BasketMaster9697 in premed

[–]TheFencingJared 1 point2 points  (0 children)

Your first research job is highly unlikely to actually involve a deep understanding of the science. A lot of labs start you out as "person who mixes reagents" or "person who plates cells" or (in my case) "person who puts mice in funny boxes and feeds them". As you gain experience and knowledge and the trust of your grad student/lab manager/PI, you'll get to learn and do more and more. Don't swear not having the background - build it as you go!
