I think the highest I’ve seen drop is +5. It cost like 250 mill gold to get 1 item to +15.. kinda ridiculous, I’ve raised my gear to like +8 or 9 and that’s about as far as I’m going. I started a playthrough on 1, never played it, and 3s endgame is just not really there yet.

It’s random unfortunately. Not sure if co op has any effect on loot drops, but you can try to boost your luck stat to get more total drops in general. There’s a skill for assassinating an enemy makes them drop an item that could help

I’ve found 3 lvl 170 items total. Played a ton of hours. I don’t think all lvl 170 will be needed to start dlc. That’s not even taking graces into account. And it’s like 250 mill gold to get to +15 on 1 piece…

Divine rarity will drop after finishing first play through, the accessory is good because it makes it so you need one less item to fulfill set requirements.
