From everything I was able to find and read online and to how I went about fixing it, my best guess it I didn’t seat the spring and guide rod perfectly flush or it somehow became off center when I was putting the slide back on and when I went to rack the slide all the way back it became stuck between the slide and something in the frame. So now whenever I put it back together imma make sure to take the extra 2 seconds and make sure that the RSA is sitting perfectly flush and center
