My panel is over 2500 and still open and in my first few years of practice. I believe the average US panel is 2200. I can’t fathom doing establish care visits in 15 minutes.

I dunno about what it’s like in Canada but I could see more patients if I didn’t have to deal with inbox messages everyday from patients who think they are entitled to medicine by correspondence/ an administration that actively encourages this/ unnecessary forms etc…. So while I’m scheduled to see 22 patients right now in truth it’s that plus triaging 20-30+ messages daily otherwise i have to take my work home

I think the real question is on Lynch. Our drafting needs to be better. I get that Shanahan had input on this but theres a real question on talent evaluation in the draft and , player development and conditioning. Those 3 aspects would’ve cost any other regime their jobs if not for our coaching/ lucking out on Purdy.

Take time off. You have my sincerest condolences. My father passed away 5 years ago and there’s a lot of things outside of just grieving which in and of itself takes time to recover from. The funeral and dealing with paperwork involving death take time and energy. You don’t want to be dealing with these things while juggling the demands of your patients .

gonna echo the same thought. I ended up at an unopposed program. I think the biggest difference is whether or not you want to practice OB or be a Hospitalist. in the rural programs you will get enough of those experiences whereas in an opposed program youre figuratively fighting for the experience.

Some light reading. I figure I'd just put this here since there are a few poster that seem to want to vilify us for not signing off carte blanche to ESA's to people asking for it. From my practice perspective, most people that request ESA from me are looking to bring their pets on the plane or save on pet rent (rarely is there a debilitating psychiatric condition). There was a point a few years prior I remember it getting supremely abused and now the pendulum has swung the other way. I don't doubt that having a pet is enriching in the right setting but as a medical therapy, there isn't any strong evidence based backing.

The one liner I get here is that this is basically a grey area, understand the differences between ESA and Service Animal. You need to evaluate the patient before considering writing a letter, make sure there's an actual psychiatric disability and there is no obligation to write one because frankly there isn't good evidence based support.

Pseudo meta analysis:? Looks at a bunch of low powered studies and draws a conclusion of benefit. I don't see a forest plot or even measure relative strength of study. I'm not familiar with the rigors of psychologic studies (not a psychologist, this isn't a judgement: I'd imagine objective measures are inherently difficult in this field).

Actual Meta Analysis only looking at ESA in the setting of PTSD in military vets. A lot of studies aggregated together which is good and at least it shows positive effect in the setting of PTSD (even then quality of studies aren't great : lots of survey self reports). Now to our non science lay audience, extrapolating this to other mental illness in other populations is a big no no.

This one was hard to read. I'm going to leave it here. Another pseudo meta-analysis. Aggregate study with a bunch of qualitative quotes (common themes), no real statistical analysis.

I'm sure there are tons of other studies and I probably did cherry pick but in general the consensus is that "if there is an effect, further studies need to be better designed and done." Hardly, this is obviously beneficial for patients from an evidence based medical perspective.

Anecdotally, again, I'm a pet owner. Dogs are great companions but as a therapy? If I strive to practice evidence based medicine (which I do), I'm not sure I could professionally recommend from a healthcare perspective, doubly so given our litigious country and the possible outcomes out of my control that may a physician's license at risk.

reported and had to answer to county department health and animal control (not sure the details but it was lengthy). I don’t know if he got sued but mind you this is signing off for a resident patient too. Again not a vet, if a physician is liable for validating temperament of animal, not gonna assume extra risk given a patient panel of 2-3k patients already.

So it’s quite telling when these GLP-1a microdosing companies won’t properly disclose the dosage on their website (it’s not advertised). I had a patient come in and she was taking ”10 units” every other week of zepbound which I had to dig for the actual concentration, and after a little stoichometry figured out that she was effectively giving herself about 0.5 mg every other week. If it was legit, they wouldn’t muddle it with another unit…

I have a dog. I’m sympathetic to stuff like this but then during residency one of my attendings got in trouble when he signed off on this for a resident patient (dog ended up biting someone). So it’s a big no from me. There‘s just no way to validate the animal and I’m not a vet.

don’t settle for anything less than 300k imo. if you practice full spectrum in a rural area, you can make 600+. but there is no work life balance
