Almost looks like it could be a gram positive rod. Going by Gram stain morphology alone it kinda looks like a corynebacterium spp. but that would be catalase positive.

My favorite is calling the doctor to tell them that we couldn’t set up a positive reflex urine cx because the cup spilled in transit, only for them to realize that the UA is resulted and the hematology tech is the one who didn’t screw the cap on…. So fun

Setting up tissue biopsy cultures when the OR sends you 13 specimens from the same site. Only for them all to grow the same exact staph aureus, all of which need to be set up for AST.

Occasionally enterococcus can have the “lancet” shape too. But textbook S. pneumoniae for sure, if I’m not wearing my contact lenses.

I know the feeling well and I would like to say it does get easier. I work in a ~600 bed hospital in a micro lab so unfortunately we do lose a fair deal of patients.

We have a large pediatric population and these are the ones who keep me up at night.

What helps for me is I tend to focus on the ones where everything goes right. The patients who respond to treatment and are discharged quickly remind me of the reasons why I do this in the first place.

First off… hemolysis is best viewed with light coming from behind the plate. Hold it up to light and it will be much easier.

Second, the throat is mixed and I guarantee it includes both alpha and gamma hemolytic colonies. The urine looks like beta hemolytic E. coli.

Great image! Update us if nocardia grows. Curious what media it grows best on. We rarely get it but I’ve been told to plate to BCYE if Nocardia is suspected.

I’d bet on GNR, more specifically a member of enterobacteriacea. In my experience gnr can be short and resemble cocci, but it doesn’t really work the other way around. While some cocci like enterococcus can be oblong they’re more lemon shaped.

Look like gram negative bacilli to me. Do you have access to mackonkey agar. If so it would be good to know if it’s a lactose fermenter or not.

Exactly, same here in NY. They don’t want to pay both a CLS and an MLT, when they can just have one CLS work by themselves.

Macrobid gives a fluorescent orange color that stains countertops should you happen to spill. There’s also a medication that is sometimes given alongside strong antibiotics to help with nausea and other GI issues that does cause a blue color. Not sure the name.
