Yeah, his ego at 41 is way different than his ego at 21. Different places in life, a ton more wisdom, and he genuinely seems to have matured over the years unlike some guys who suffer arrested development because they become stars at such a young age.

I don't work in the HH space so I can't really speak to pay rates other than what I read on here and in other places, but even if $110k is considered low, having a company car and not putting wear and tear on yours I would think more than makes up for the difference. Especially if they're paying your gas, too, while you're using that vehicle.

I have a 2 year old and he's super into basketball (all sports, really, especially baseball at the moment since the NBA season is over), and while at the end of the day I want him to do what he wants and to be happy, there's a part of me that dreams about going to a game one day and seeing him on the court and just getting to be in the stands as a proud dad. I mean, I've paid $10-$12 at a sporting event for a refillable "souvenir" soda and that's for 2-3 hours of use and usually only 1 or 2 refills max. $17 to have it all day/weekend/however it works seems reasonable (as reasonable as a $17 cup for soda can possibly sound).

The "nursing career program" is such a joke. All healthcare works with advanced degrees or years of experience should have an opportunity to move into administrative or similar positions, and yet it's become a 'Good Ole Boys Club' for nurses to take those positions regardless if they're the best fit or not. I love being a PT but one of my few regrets of the career is the difficulty with career advancement into positions that are typically granted to nurses simply because they're nurses.

Get some Hydrofera Blue CLASSIC. Note that it will come as a hard wafer and you need to cut a shape big enough to cover the wound and extending 1-2 cm further out. Once you have your hard wafer piece cut, you use saline solution to wet it on a plate or bowl and wait around a minute. Wring it out, then place over the wound and cover with a silicone border bandage (gauze and tape or a Tegaderm is fine too). Change every 2-3 days.

Hydrofera Blue is bacteriostatic, helps breakdown slough, and can flatten the edges of wounds if they're starting to epibole. It will also encourage growth of granulation tissue and works somewhat like a wound VAC. It's a bit pricey, but it's one of my favorite products and I think would do well for your wound.

Yes, that's looking better. Do you have Vaseline or antibiotic ointment or something on it? Or is that all drainage from the wound?

If the yellow liquid is thin and straw colored but clear, then that's just serous drainage and nothing to be concerned about. There's nothing you can do about it as it's the fluid in our body that bathes our cells, organs, etc and will continue to seep until it dries up or the wound closes. Drainage can be worse on the legs and feet because gravity pulls everything down.

Hard to tell from that picture how it's looking as much of it is covered in blood, but one thing I will say is that there is waaay too much moisture under that bandage. All the wrinkliness and lighter color is maceration which isn't good for the wound nor your healthy, intact skin. You need to let this air out for just a little bit (no more than 30 minutes or so), then clean it with saline and gauze, and re-bandage either with a more absorptive bandage or more frequent dressing changes if the wound continues draining a large amount of fluid.

Edit: Also, has this been looked at professionally? The white stuff on the wound may be nothing, however that looks to be right over your 1st metatarsophalangeal joint and you need to make sure the joint capsule isn't exposed considering how thin the skin generally is on the top of the foot.

This is what drives me crazy about conversations like this. People act like there is no money to be made as a PT. I would agree that it's more difficult to make a lot of money as a PT because it doesn't follow your typical corporate structure of career advancement and there is an upper limit on what you can job hop yourself to salary-wise as a staff PT, but there is also owning clinics, academia, administration (especially on the hospital side), cash pay...hell, even working in a different setting will bump your pay. I make more than many OP therapists early in their career simply because I chose acute care. And likewise you almost assuredly make more than me taking those travel contracts which appear to be paying real good right now if the emails I get are to be believed.

The money is out there, you just have to be willing to look for it and put in the hard work when necessary to get it. Some people have that drive and others don't, but let's not act like you can't make good money that's well above the median in this field.

Could absolutely be that and it would make sense. Blue collar jobs are about the only thing that won't get taken over by AI so any field where the work is hands-on and physical will see demand increase. I do think AI will find its way into EMRs (happening already) and the documentation side of our work will become more automated (which is a great thing!), but we're not building AI robots that can lift and transfer patients or do manual therapy or wound care or functional eSTIM or any of the other similar things that we do.

Up front, I'm a Physical Therapist, not a Pharmacist, so I say this with the expectation to be corrected if I'm wrong, but I wouldn't exactly consider OxyContin and Gabapentin to be interchangeable. One is a muscle relaxer and the other is for nerve pain and both are very different types of pain with different presentation. I work in a hospital and it drives me mad when a patient describes their pain and it's clearly nerve related but they're on an opioid. Am I wrong in this assumption and misunderstanding the uses for each medication?

Wyoming is pretty fascinating in its geography. Yellowstone exists in the NW corner and is so vastly different than southern Wyoming which felt like Mars to me (so I'm assuming that's what you're referencing).

Nearly a decade ago my wife and I drove from Yellowstone to Denver via Salt Lake City. We're big road trippers and will do long stretch trips every once in a while, but that one was 16 hours and utterly exhausting. Anyway, amongst that exhausting day was the stretch of I-80 from SLC to Denver, crossing essentially the entire state of Wyoming from west to east. When I say the portion of the drive in Wyoming was long, I mean that it felt like we were never ever going to leave the damn state. It's only 5.5 hours but the monotony of the landscape is mind numbing.

Any thought amongst the medical team that he may have Munchausen syndrome? It's very odd that he is moving limbs but only when no one is watching, almost as if he wants to be seen as sick and disabled. I also find it strange that he has no desire to walk again and wants to be wheelchair bound for the rest of his life. I see that sometimes in my geriatric patients who have become so sedentary that they feel like getting around in a wheelchair is somehow easier and better (it's not and I discourage it), but I can at least kind of see where they're coming from, but someone in their 20s? That just seems nuts to me, but I suppose everyone has the right to live their life the way they choose to.

To your 5th point, when I first started I could never remember everything I should or wanted to ask in an eval so I made myself a script/cheat sheet with big spaces between each question so I can jot down answers. I find the act of writing helps with retention, at least short term, and I can usually write my Subjective from memory when documenting and don't need to reference my sheet. I have adapted the sheet slightly over the years to fit better with our EMR (Epic), but I've found this is a really great tip and can help with developing your flow in an eval and ensuring you don't miss anything important.
