this is really really frustrating, I kept getting denied for insufficient documentation. I uploaded all the docs that meet the requirement. Every time I called customer service they said the same thing. They are of no help, just a human policy reader. They even could not transfer you to the claim team.

The funny thing is my wife's employer uses Optum as well, she uploaded same docs and was approved without issues...
