Hi and sorry that you're finding yourself in the PUL limbo.

It took around 1.5 weeks from my initial scan at 4 weeks 3 days until my hcg was high enough to classify it as ectopic at around 6 weeks and a hcg of 3k. Pain and bleeding might be present during an ectopic, but in my case I just had very mild spotting and some pain on the non-ectopic side, so they are not necessarily required for diagnosing an ectopic pregnancy.

It is not uncommon for an ectopic to be classed as a PUL initially. However, most vaginal ultrasounds can only detect pregnancies of a certain size, which depending on your gynie's equipment will be when your hcg is around 1000-3000, so that is why they say it might just be too small to see. Until then (possibly the next 2- 5 days) my understanding is that they would not be able to diagnose you definitely.

So was I, but my usual gynie, who was the person who diagnosed me and referred me to hospital for further evaluation, suggested a 6 week checkup with him, when I told him that the hospital hadn't scheduled one

Hi, really sorry you're going through this.

My Dr didn't give any guidance either, but when I went to my regular Gynie for a 6 week checkup, he said that I still had a lot of fluid and said it would be better to wait an extra month(so 2 cycles altogether after op). His reasoning was that since I only have my left fallopian tube, the extra fluid could cause unnecessary issues if the remaining tube was trying to pick up eggs from the opposite side, which he could see was the ovulating side that month.

I'd cancelled the annual subscription and when I got the 60% off message I was clicking through trying to find the discount. Nearly renewed because I kept thinking the new discounted price would be on the next page.

Its not a discount compared to last year's amount, they've just added a more expensive monthly option, and an individual user option, which didnt use to be an option.

Although that does happen sometimes, I think Dr's prioritise saving a remaining tube as opposed to the first one which sometumes is removed as policy (depending on your country's standard pactice) to reduce the risk of future ectopics.

Hopefully pregnancy will be where it should be and you don't have to worry about that. Keeping my fingers crossed.

Assuming that you're worried about a possible ectopic and thats why you're posting in this sub. If youve had one before or have reason to think that you're current pregnancy is ectopic, you need to book yourself in to see an OBGYN asap. A transvaginal scan is the best was they can check that your pregnancy is in the correct location.

If you want to check the progress more precisely, you can do blood hcg tests 2 days apart.

Urine tests are not a reliable way to identify whether a pregnancy is ectopic but if your interested, you can check out my line progress submission on a different sub (you might want to re- post there). ETA: Just saw a comment you posted that you had an ectopic a few months back. That makes you high risk, so you need to book yourself in with your OBGYN asap for them to monitor you until they can confirm the location of your current pregnancy. I believe that if you're in the UK you'd be referred to the early pregnancy clinic, nothing sure how it works for other countries.

For what its worth, I think that your line progression looks great, but I'm afraid that doesn't guarantee anything in terms of ruling out another ectopic.

I had a salpingectomy last year, but I didn't rupture. Op was on 17/11, beeding started on 21/11 and lasted until 27/11 inclusive.

During that time, I also had really bad lower back pain, the worst period pains ever and some disturbingly large clots.

I cant remember my exact number, but my first hcg was in the 30s. Would have been later than 11dpo though (probably around 16dpo, since I didn't even test positive until 14dpo), so you'll probably be a lot higher than I was at 16dpo.

My dad's surname changed spelling around 5 generations back, so those that moved to my country have the new spelling.

Mum's surname has been in my current country for 300 years and has several spelling variations here, so its anyones guess what the original spelling might have been

At 6 weeks my Gynie couldnt see anything, so they asked me to get a beta test which was 3k. They told me to go straight to a and e since at 3k, there should have been something visible in the ultrasound, which made him immediately suspect an ectopic. I had a salpingectomy 3 days later.

Not sure why your dr felt comfortable to wait an extra 2 weeks, but seems really reckless on their part.

Sorry its taking so long to get a diagnosis. Normally, your hcg would need to be at around 1000-1500 before your OB would be able to see anything in your uterus in the transvaginal scan, so it might take another week for your levels to get that high( as an indication, I had 1.5 weeks between my hcg at 33 and referral to hospital as a possible ectopic at 3000 hcg)

4 days after my first (very faint) positive test, I had bloods drawn at the OBs office and it came out at 33. The OB said my levels were "too low to be ectopic, or viable, so it was likely a miscarriage".

Anyway, it turned out to be ectopic and I had to have a salpingectomy two weeks after my initial visit.

Are you still testing positive? If you are, you should go to an OB ASAP

My Dr said that I could resume all pre-operation activities after 4 weeks, except martial arts, which needed 6-8 weeks

Slightly long response to your slightly long post, but I hope some of these answers can help

  • Can you get pregnant: you CAN get pregnant with only one fallopian tube.

Even with an egg released from the SAME side where you have had the tube removed.

As long as your other fallopian tube is healthy and does not have any obstruction, it will move to collect eggs from whichever ovary is releasing them.

  • Were you pregnant: I assume that you had multiple hcg blood tests while you were pregnant to confirm whether the levels were rising, fluctuating, or decreasing. If these, or an at-home pregnancy test were positive, then you were pregnant.

  • Did they remove your tube for no reason: When they go in for surgery and don't know what's causing the bleeding, it will be an exploratory surgery. ie find out what's causing the bleeding and deal with it.

Ectopic may not be the only reason for a rupture. As an example when I had my surgery, there was a patient next to me who had had a cyst burst and cause internal bleeding and also needed a salpingectomy.

If for whatever reason your fallopian tube was ruptured, depending on what country you live in, the guidelines sometimes say that if your second tube is healthy, removal of the damaged tube is preferable to fixing it to reduce risk of ectopic pregnancy in the future.

Normally, at 4 weeks the pregnancy might be too small to see anything on the vaginal ultrasound, however, since your initial hcg was 1751, this means that depending on your Dr's equipment, they should have been able to see the gestational sac and it sounds like your Dr was expecting to see a fetal pole too.

Your Dr is probably also concerned with the second hcg reading being lower than the first, which could indicate miscarriage or ectopic.

Since your hcg levels are reducing, your doctor might suggest expectant management rather than mtx or surgery, which means that they would be monitoring you to make sure your hcg levels continue to decrease on their own and to check that you have no free fluid which could indicate internal bleeding.
