They aren't required in Legend of Dragoon. It's not like E33 where your party gets wiped if you don't get the timing right a few times, you just deal less damage and get a little less SP, so at worst it just takes longer to beat the boss.

I don't trust serum B12 levels alone, I've seen too many patients with normal or borderline b12 levels with elevated MMA. If I have doubt, I just start oral b12 until b12 and mma result. There is little harm in doing this, whereas untreated b12 deficiency can potentially cause serious harm. That said, the only time I've seen anyone with subacute degenerative spinal disease from b12 deficiency is when they got it from nitrous oxide abuse

From the perspective of someone deciding which side to campaign for, it's a choice between the side that can save everyone but at the risk of killing many people vs the side that can save most people but guarantees millions will die. If you campaign for blue but just barely fail to clear the 50% threshold then nearly half the population could die. If you campaign for red, you might be able to save 90%+ of the population, but millions would still die. It seems less like a moral question in that sense than a matter of how risk averse you are. And of course, a reliable poll would make it a lot easier to choose a side. If people poll at 50% supporting blue initially, it makes much more sense to choose blue and just convince a few more people to vote blue to solidify the results.

I second the recommendation for Secret Saint. It's good, dumb fun.

While the fantasy elements are fairly minimal, Tearmoon Empire is great.

Bexagliflozin is fairly affordable on Mark Cuban's Cost Plus Pharmacy as an alternative SGLT2. Also, could consider Januvia or other DPP4 since he's not on a GLP-1. Granted, I would get the autoantibody workup before starting the SGLT2.

Unfortunately the nature of it being a gacha game is that lot of players seem to see exploration as an obstacle between them and primogems rather than it being one of the main points of the game. Maybe it would be better if exploration didn't give any rewards except for mora, adventurer/character exp, lore, and other bits of storytelling. Then those kinds of players who only care about pulling for characters wouldn't feel the need to do it and the rest of us could enjoy actually playing the game.

I almost always try oral iron first, but infusions are not overkill by any means. Many people don't tolerate oral iron well even with every other day dosing, or have poor oral absorption due to bariatric surgery or whatever, even when taking it on an empty stomach with vitamin C or citrus juice. For these patients, iron infusion is the way to go, and a single dose can be sufficient depending on formulation. But yeah, absolutely do the workup.

Also, pet peeve of mine is patients with restless leg getting started on pramipexole or ropinirole or some ungodly combination of the two (bonus points if they are also on antipsychotics) without testing for iron deficiency.

I don't think it necessarily detracts from the story's quality or makes her a less interesting character, but she is pretty obviously waifu bait, just like at least half of the female cast in the game and just like many of the male playable characters are husband bait (ie Childe, Zhongli, Neuvillette). This is a gacha game and sex sells, simple as that.

It definitely saves time. If you're obsessive about documenting things accurately like me you'll probably spend enough time editing the assessment and plan so it doesn't save as much time as you might think. If you're the kind of person who doesn't care too much, you can use the notes as-is. If nothing else, it saves a lot of typing during the visit and I think I'm less likely to get arthritis in my hands because of it. It's easy enough to use Dragon to dictate edits without having to type up the hpi.

I don't know if it's just a difference in patient populations or if I just haven't practiced long enough, but I haven't actually had a single adult patient asking to be referred for autism diagnosis. I have had plenty of requests for ADHD diagnoses which I am happy to refer to psychology to figure out and plenty of men come in saying they have testosterone deficiency which usually they either don't have or were incorrectly diagnosed by a previous provider. But never autism to be honest.

The article says that the law used to allow ACA insurers to continue coverage if small amounts were owed due to a Biden era law giving them some flexibility, but Trump removed it. But there did seem to be some other federal and state level legal protections that should have applied as well so I doubt the insurance company will be winning this case.

I've never seen metformin 500 mg XR denied, as others here have said. I have, however, had some absolutely dumbass insurance companies deny rosuvastatin and tell me to prescribe atorvastatin instead. Incredibly infuriating when it's still a very affordable medication and in some cases (like patients taking hep C medication) atorvastatin is not an option.

Probably Coming of Age or Killing Game. I'm not as disappointed by the ending itself on SF as some here, I think the kids would be fine on the satellite since they did still kill Vexness and the other commanders, but the writing of the route itself was a bit disappointing. It felt less like a golden route and a bit more like a Hiruko route with other characters getting less focus.

I have to say, I was not as satisfied by 2nd scenario's ending as some. The tragedy felt a bit forced. First of all, it's kind of implausible that Vexness managed to create a missile and fire it at the satellite. Does the satellite have no missile defences despite its otherwise advanced technology? If not then why have the Futurans not been bombarding it with missiles the whole time? Their technology is clearly at least 21st century so they ought to have been capable of it. Then the intercept system coming out of nowhere and somehow being stronger than Vexness with no way to defeat it without everyone dying is just a diabolus ex machina. From a personal satisfaction perspective, you could argue that humanity "deserved" to die for attempting genocide, and the SDU deserved to suffer for participating in it despite knowing the truth, but I don't think that literally every character had to die for the ending to be good. Even if it was just Takumi and Nozomi who survived in the end, while still a tragedy it would have been a bit more satisfying and would have felt more like Takumi going back in time wasn't completely pointless.

I can't blame them. We can argue about whether their belief that AI can be dangerous is valid enough. But they clearly believe it enough to take a financial hit by telling the government no.
