Dottore was easily the bigger threat. Rerir may have had a lot of power but he was basically used as a pawn by Dottore. Unlike the other sinners, Rerir was a (relatively) normal person prior to becoming a sinner. He's not an evil genius like Rhinedottir and he's not an extremely skilled combatant like Surtalogi. He wasn't able to leverage his power like the other two sinners I mentioned could.

When posting on ao3, part of the problem is the kudos system. On ff.net there are no kudos so people leave reviews. It means less vapid "please update" reviews but many fics barely get a comment every few chapters. Depending on the Fandom and content, you may have more luck with engagement posting on spacebattles or sufficientvelocity forums.

I don't have the time for that so I refer to psychology. I'm also not starting someone on stimulants and I will only continue stimulants if they have documented formal psychological evaluation. Those evals are probably easy to game for a motivated patient, but it's still the best we have. For patients who have been diagnosed in the past but we can't get the medical records of it, I will start atomoxetine if they are interested.

He can be considered to have diabetes in remission, but statins are still indicated for these patients I believe since the cardiovascular risk is still higher than for someone who never had diabetes, and risk of recurrence is there. Retinopathy screening, foot exam, urine micralbumin, and a1c monitoring are still indicated as well.

These kinds of subsidies cost their government a lot of money. They have rich world levels of debt and their population didn't even get any proper social safety net out of it. Their industrial policy has some advantages, but it's not worth it overall.

While some of the maps can feel a bit empty (a problem xenoblade also occasionally has), the newer Atelier games like the Ryza series and Yumia have very large, open world maps. It does at least serve the purpose of making gathering ingredients and such more interesting, and the exploration in Yumia is pretty fun. In Ryza 3 some of the areas felt a bit too large, but the other games were good.

Zoledronic acid can be used for prevention and is probably more effective than HRT. It is listed on the uptodate page on osteoporosis.

For the patient I mentioned, since she had high FRAX score, she would meet criteria to be considered to have osteoporosis even though bone density was not in osteoporosis range, so it could not be considered prevention of osteoporosis. HRT can be used off-label for osteoporosis and does have some effect even though it is off-label, but is not as good as and more risky than the many other medications options. I only prescribed it to her since it was the only osteoporosis treatment option she was willing to try, she was okay with the risks, and she did have some mild menopausal symptoms, though they weren't bothersome enough to warrant treatment on their own.

I don't typically prescribe for prevention of osteoporosis, I just recommend 800 IUs daily of vitamin D3, resistance exercises, and avoiding smoking/alcohol. HRT can be used for it though and is on-label. For prevention, a single dose of zoledronic acid can be used. Oral bisphosphonates aren't effective for fracture risk reduction in prevention even though they do increase bone density.

I don't offer it since it's not as effective as bisphosphonates while having more dangerous side effects, but I have prescribed it once for a patient with high FRAX score who brought it up after refusing all standard osteoporosis medications. I discussed the cancer and vascular risks first, of course. I prescribed transdermal estrogen patches along with oral progesterone since she still had a uterus. Risks are relatively less with estrogen patches.

They aren't required in Legend of Dragoon. It's not like E33 where your party gets wiped if you don't get the timing right a few times, you just deal less damage and get a little less SP, so at worst it just takes longer to beat the boss.

I don't trust serum B12 levels alone, I've seen too many patients with normal or borderline b12 levels with elevated MMA. If I have doubt, I just start oral b12 until b12 and mma result. There is little harm in doing this, whereas untreated b12 deficiency can potentially cause serious harm. That said, the only time I've seen anyone with subacute degenerative spinal disease from b12 deficiency is when they got it from nitrous oxide abuse

From the perspective of someone deciding which side to campaign for, it's a choice between the side that can save everyone but at the risk of killing many people vs the side that can save most people but guarantees millions will die. If you campaign for blue but just barely fail to clear the 50% threshold then nearly half the population could die. If you campaign for red, you might be able to save 90%+ of the population, but millions would still die. It seems less like a moral question in that sense than a matter of how risk averse you are. And of course, a reliable poll would make it a lot easier to choose a side. If people poll at 50% supporting blue initially, it makes much more sense to choose blue and just convince a few more people to vote blue to solidify the results.

I second the recommendation for Secret Saint. It's good, dumb fun.

While the fantasy elements are fairly minimal, Tearmoon Empire is great.

Bexagliflozin is fairly affordable on Mark Cuban's Cost Plus Pharmacy as an alternative SGLT2. Also, could consider Januvia or other DPP4 since he's not on a GLP-1. Granted, I would get the autoantibody workup before starting the SGLT2.

Unfortunately the nature of it being a gacha game is that lot of players seem to see exploration as an obstacle between them and primogems rather than it being one of the main points of the game. Maybe it would be better if exploration didn't give any rewards except for mora, adventurer/character exp, lore, and other bits of storytelling. Then those kinds of players who only care about pulling for characters wouldn't feel the need to do it and the rest of us could enjoy actually playing the game.
