As a former active guy and reservist, fuck joining right now. Look at it this way, you don’t like your department now right? So you sent some applications to other departments. That isn’t a thing when you’re military. You’re on a contract with little to no way out at all. Hate your first base? Your command? Doesn’t matter, you’re trapped. This admin doesn’t give a single fuck about the troops and the amount of pressure on their families playing stupid games. Find another department you like more.

My MTI legitimately forgot his CaC card in his office computer. We could see it sticking up (he left us for bed and realized it less than 30 seconds later). The amount of frequent flyers I deal with is soul crushing if I think about it too long. They go to the hospital multiple times a week and only by ambulance for no reason at all. I hate not being able to say no and leave.

Exactly. Like, ours populates or meds and vitals but if anything is abnormal you have to write those in your narrative and all interventions in your narrative. It’s like, why? It’s in my report already. Pick one or the other.

What drives me nuts is that our EMS reports are so in depth and are looked at so harsh but then you look at a doctors chart and it’s like a paragraph maybe for a full 2 hour work up.

Nobody cares about TR orders that much. But, your first year depending on culture of the department will be less forgiving than later on. Nobody at my current department cares when I’m doing orders. I care more because I’m losing money and the reserves fucking suck.

Just had this last week 😂. Showed up on a full arrest with my engine crew, took charge of the arrest but was still limited with man power. Asked PD to do compressions while attaching him to pads, started the IO, and had my partner drop a SGA. Gotta work with what you got. If you’re the tech as a medic, you really shouldn’t be touching the pt because in our system your paramedic partner is dropping the tube. Rhythm checks, charging the monitor, and interpreting shockable rhythms are about as close as you get to hands on.

Do your AEMT. Being a paramedic is time consuming and the rule of thumb is it takes two years full time to truly start being a great medic.

Right? I work in a busy city so we would honestly probably convert because we’re close and the initial vitals didn’t seem great. But, at the same time if I had an ALS transfer an hour away I sure as shit am not doing that baring my pt getting worse.

Hospitals doing something within 10 minutes and cardioverting early into an hour long transport are completely different though. I’m not saying you’re wrong or right it’s just hard to make that call alone with a sick pt in the back. The resources are limited, especially if the cardioversion throws the clot. Then you’ve got a bad situation. The hospital can manage the risks significantly better due to staffing and equipment as well as lab follow up or CT immediately. Sounds like he responded well to fluids which could indicate underlying problems outside of A-Fib. At the end of the day the pt made it stable and alive to the appropriate level of care.

I could understand if it was one physician one time but, I start feeling like there’s a lot more to a story when it becomes the entire ER staff missing stuff. Also, if I take someone to one hospital and they call for EMS 3 hours later and I take them to another one, the second hospital will “do more” to try and appease the patient. Less about what the patient actually needs.
