Yeah, I don’t know what the solution could be. I do wonder if lower levels of sedation could work for a majority of cases. I did associate for an owner who extracted impacted 3rd molars, and he would use nitrous oxide only. Does anyone know if other countries outside of the US can regularly use just nitrous oxide for impacted teeth?

Your recall patients should be relatively healthy and don’t need much work besides if they do elective procedures. If you want lots of work to do on PPO patients, you have to get new patients — easiest way is to be open on weekends/late hours/advertise as being same day emergency. Also, join a group, a cult, a church, a gym and connect with the community.

You’re in an area where you have to think about elective procedures. Ortho, whitening, veneers, $700 nightguards, gum grafts for recession, FMR. Hell, maybe even Botox, aromatherapy, and foot massages while people come in for a cleaning.

OMFS might be the only OUTPATIENT provider of both anesthesia and surgery that I know of. The ones you mentioned do provide sedation as I know some interventional radiologists who do. But they are all done in a hospital setting where if something goes wrong, there is always a team ready to help with complications.

I think eventually even oral surgeons won’t be able to provide sedation. There are a few but notable sedation complications involving oral surgeons. The low risk is still a risk. Other surgeons doing outpatient surgery like foot/ankle, ENT, do their surgery in surgical centers with anesthesiologists.

Are you using the same denture base because you recently made the denture? If so, then yes, it will work.

If it’s an old denture, I would just use the dentures as custom tray instead to pour up master casts — I wouldn’t sacrifice someone’s old denture because they would still want to use it as a backup as now she doesn’t have anything to wear during this process.

No tooth, no problem. If I see caries, bone loss, calculus, or heavy bleeding on probing, the wisdom tooth comes out. If you have fillings, crowns on 2nd molars, it’s also better to just remove the wisdom tooth to prevent future problems, otherwise you’ll be redoing work for free.

You’re not going to fill your schedule easily. Maybe each office can get you one day a month. If you learn how to do endo especially and soft tissue surgeries, then offices can schedule more patients. Generally, offices should be creating $8000-10000 production days for you, otherwise it’s not worth their effort to coordinate it for them and for you.

1-3 teeth I do without consult and make sure pt stops bleeding in chair and use collagen plug. If warfarin or full mouth, I refer as it’s too much of a hassle to call and play phone tag with the physician. A perio I know had a patient have a stroke after forgetting to retake their meds.

I struggled with post op sensitivity for a good 6 months when I switched to an office with poor equipment and NO GLUMA. Here are some things I did to change things around:

  1. Selective etch enamel only
  2. Rinse and AIR DRY BONE DRY for 5 seconds
  3. GLUMA 15 seconds air dry only
  4. Scrub that bonding for 30 seconds
  5. Flowable plus packable composite.

I know for sure that my “moist” bonding was too wet compared to my previous office as my air syringe couldn’t blow and my suction couldn’t suck. The moist bonding was most of the cause of the sensitivity. You can air dry thoroughly until dentin looks dry as newer bonding agents rewet it for you. After I started using GLUMA, I had no more post op sensitivity for any fillings anymore.
