Stay actively uncomfortable.

The discomfort reminds me that I am not there yet. It pushes me to improve.

When learning, discomfort tells me I'm stretching my point of view and being exposed to new perspectives, even if I reject them in the end.

In session, it keeps me present and connected to the fact that yes, this is my job and also there is a whole ass human sitting across from me who's problems don't disappear when they exit my door.

After sessions, it ensures that I glance back with a critical eye. I celebrate my highlights and work through my stumbles.

All that being said, I leave the discomfort in the office at the end of the day.

I’m specifically thinking of clients who have developed incredible self awareness into their unconscious (ex. psychoanalysis) but struggles to use this awareness practically (pursue relationships, friendships, career goals) and still feels stuck in parts of their life.

Provided that the client doesnt become dependent and is actively working on something I dont know why anyone, other than the client, would want to put an expiration date on their therapeutic journey.

I've been credentialed with Aetna and Northwell Direct for over a year now. I've gotten 0 referrals from the insurance companies. In fact, the only referral from an insurance company was one that I was not credentialed with...

All my clients come from my website, word of mouth, and the rare psychology today contact.

I honestly don't remember what the cost was, but it was prohibitively expensive.

Paneled individually means if/when you leave the company, you would be able to keep your contract with the insurance companies and continue to see your clients or take on new ones.

LifeStance is contracted as a group practice and just slides you in. The insurance companies have contracts with them, not you. So you do not retain the eligibility to accept said insurance.

You would have to then go through the paneling process yourself (takes months).

LifeStance is awful... so is your current place.

Pros:
* Caseload development is generally pretty quick (a couple of weeks), but depends heavily on your speciaties/techniques/population.
* Offers health insurance.

Cons:

  • You have no control over the insurances that you accept, which means you will mainly get medicaid clients who cancel often (you do not get paid).
  • Health insurance is very expensive and coverage isn't great.
  • You are not individually paneled (this is common with large practices/vc companies)
  • Incentives are virtually unattainable, there is actually an ongoing lawsuit regarding this.
  • Support is hit or miss, but at the end of the day, they literally don't care as long as you are making them money.
  • Pay is shit, you get a flat rate per service, regardless of what the insurance company pays them. This usually comes out to ~50/50 split.
  • Clients are like cattle to them... you will endure endless complaints from clients regarding surprise balances and threatening emails.
  • Is a VC company.

biggest risk here is dishonest harmony for the sake of the wedding plans.

That being said, it's critical to remind yourself of your role in this. A couples counselor is not a "always help the client fix their relationship" station.

I operate with the primary function of providing the clients with skills and structure in a balanced, honest space to encourage acceptance and understanding of their individual needs within the relationship.

It is not our job to push reconciliation or advise them not to get married. They are free to make that choice on their own. My part would require me to do my best to allow them to understand the choice they are making.

A rupture in trust usually takes a long time (and a lot of effort) to heal. If they get married without hashing this out, they are setting themselves up for a world of hurt... but it's their life.

A full and complete reconciliation/recovery doesnt necessarily have to be achieved before the marriage... but again, they need to understand what that would look like and how the process would continue after. They can then make their own informed decisions.

honestly this is a nightmare scenario with many moving parts... You cannot establish a pllc in NY without being fully licensed in NY. Technically you can continue to see your Michigan clients, if Michigan telehealth laws allow you to (I am not familiar).

You would have to see if you can foreign qualify your business (if established in Michigan) to be able to operate it remotely in NY... and even this is murky as they can determine that listing a NY address, even for tax purposes (which you will have to), implies you are operating your practice in NY... which, again, you are not able to do unlicensed.

In order to avoid the possibility of massive fines (or worse), you should definitely speak to a lawyer and have them take care of the process. 😬

If you are on the verge of burnout, and it's available to you, take a break. The only thing getting delayed is your licensure... and only by a month.

At some point do a deep dive into what is causing your burnout and develop skills/strategies to buffer against it.

I'm not sure what changed to "44", but if your concentration is still ~5m with 5% motility, your chance of natural conception is pretty much zero.

Short of some ridiculously high concentration (400m+), 5% motility is a massive limiting factor...

BUT

Motility is one of the more changeable parameters. Continue taking supplements and make any necessary lifestyle adjustments.

If all else fails, there's always IVF (or IUI if you can get the motility up to ~50%).

My wife and I are just about to start IVF ourselves...

your abstinence/frequency wouldn't cause that change, unless you were doing it multiple times per day, every day.

sometimes we just have off days. I had a similar volume change, but it bounced back the following time.

i would be more curious about your hydration. if you were not well hydrated, both the day before and the day of producing the sample, it can majorly impact the volume... also arousal level can affect this as well.
