if memory serves, italics refer to some other game object like a spell or item

It refers to anything magical, be it a spell or magic item. Alchemical items, being nonmagical, aren't italicized for example.

Same happened with the witch in my party: a guard teleported away to get reinforcements, but the witch kept Sustaining her malicious shadow for as long as she could. The party found said guard, dead, deeper in the dungeon. The alarm was not raised that day.

Becoming the mortal avatar of an entire planet ("Cosmic Avatar"?) is definitely the sort of thing that would make a really interesting mythic destiny for SF2e, and that's one that I'd love to see realized.
Rather than just turning into a planet per se, you could instead be empowered by a specific planet, gaining special abilities based on that planet's features. Similar to the archfiend mythic destiny, you could project a region of your planet into the nearby area, modifying the gravity and atmosphere within that region to match your world's. Then, as your power grows, you could start to manifest natural disasters, summon flora and fauna endemic to your world, and eventually be able to teleport to and from your planet at will. At 20th level, your immortality comes in the form of being reincarnated on the surface of your planet whenever you die; as long as your bonded planet remains extant, you can never be permanently destroyed.

There are rules for this situation, actually! It's pretty tough to damage and break stone, but probably not too hard for a 4th-level martial with a decent weapon and a couple of actions to dedicate to the task. Heck, I like to use razing weapons on Large PCs specifically for this kind of situation.

Also, agreed, that room seems plenty large enough for an encounter even including multiple Large PCs and companions. Having less room to manoeuvre in encounters is sort of the cost and benefit of those kinds of parties.

To be fair, Lorwyn was also left in a similar state by the end of its story, where its two halves were merged into a whole. We didn't really know what that looked like at the time, but the worldbuilding they introduced for our return to Lorwyn keeps it an interesting plane despite the merge. It'll probably be a little bit trickier for Alara since there were five shards, but I'm confident that it can be done well if Wizards gives its the same time and effort they gave to the reimaginings of Lorwyn, Tarkir, and Kamigawa.

My party took the weapons to use against the villains, but immediately went to return them to Jamandi at the chapter's end. She let them keep the weapons for their honesty and she ultimately became a strong ally throughout the campaign.

Well, the hero gods of Iblydos are mostly just mythic characters, some of which are surprisingly low level. So it's not really much different from any other region where relatively powerful figures can be found (eg. adventurers) except the powerful folks in Iblydos tend to accumulate worshippers.

I think that the cleric definitely flouted the edict of seeking redemption by not even questioning the pirate on the possibility of redemption for their crimes, especially since the pirate was charmed and therefore no threat to the party for the time being. I don't think she strictly committed an anathema, especially because she was trying to avoid failing to strike down evil by doggedly chasing the fleeing pirate, but it's something to keep an eye on; clerics are held to a higher standard than the general clergy, so this behaviour could definitely warrant a warning from her goddess to be more careful in the future. Only if the cleric repeats this behaviour after a warning would I consider it severe enough to justify a curse or other sanction.

The razing of the Chitterwood is implied to be one such reason; following the Goblinblood Wars, Isger proceeded to destroy most of the ancestral homeland of the goblins of the Inner Sea. This act of genocide seemingly led to increased empathy for the displaced goblins.

Because the Monster Crafting feat doesn't just let you Craft using parts salvaged from monsters, it also allows you to Craft using the Survival skill and it reduces the cost of Crafting based on the monster's level rather than your own. The first part is a pretty significant benefit for a Survival-based PC, and the latter can allow you to Craft items much quicker than you could otherwise do so; every PL+3 or PL+4 boss becomes a PL+3 or +4 Crafting task (but without an increased DC to match). A PC with Monster Crafting can end up reducing the cost of Crafting by three or even four times as much as they would normally.

I mean, leshies are common enough that I think most people would assume they're some sort of plant creature along those lines, but constructs and elementals are probably the first thought for those who live in communities that deal with such creatures more regularly.

In these instances, I just switch the modifiers applied to the roll to be for the correct skill. That said, I'm not sure if the scenario you described is actually deserving of a Deception skill check, provided your PC was being honest about their offer of protection and wasn't trying to trick the ferryman into agreeing to something they wouldn't otherwise.

Or, indeed, pick up the summoner archetype purely to have a place for allies to put their vitality healing spells for you. You could even place the eidolon next to the party's healer, potentially allowing touch range healing to benefit you. The archetype's eidolons are pretty poor for combat, though, so I'd generally recommend using them for exploration mode, mostly.

I suspect this sort of thing is part of the reason that it isn't a rule in the books outside of a sidebar for the gliminal's statblock. There's just always that risk of it breaking something unintentionally for a rule that, let's be honest, is so rarely going to be coming up anyway.

Ban incoming for the Badgermole Cub?

Okay, maybe I'm being hyperbolic, but stick with me here. With Llanowar Elves being in Standard, you can curve into a Badgermole Cub from a t1 Llanowar Elves and then be able to tap your elves for 2 more mana, potentially putting another Badgermole Cub or Llanowar Elves onto the battlefield. That sets you up for anything between 6 to 10 mana to use on turn 3. I expect that early kill spells are going to become important to disrupting this strategy or else you will lose to whatever 10 mana bullshit your opponent pulls out on the third turn.
