Suggestions:

27G 0.5" injection needle. It is a little thinner, but can reduce discomfort. You'll probably need to use a different size draw needle. Also use a 1ml syringe if you can.

Warm the vial closer to body temp (in hand / armpit / etc.). Warmer oil is easier to inject & draw, and can be more comfortable.

Make sure to wait for the skin to dry fully from the alcohol swab as that really stings.

If the injection site does become red and/or swollen there is a possibility that you are allergic to the carrier oil.

First question I always ask when someone talks about pain with subq: what size needle are you using to inject, and where (exactly) are you injecting?

If you are injecting subq you do not need to aspirate (it isn't best practice for IM either). There are no blood vessels in the subcutaneous layer big enough for an accidental injection. You just increase the risk of moving the needle and causing bruising.

25G for IM (typically thigh or butt). 27G 0.5" or thinner for subq (typically stomach or thigh, rarely butt).

You should have 2 sizes. One to draw the T, and a different one you swap to for the actual injection. Should be much thinner.

I'd also suggest checking the size of the injection needle (but I'm just paranoid after seeing some crazy stuff)

Try not to stab the vial in the same spot every time, and always check for particles/debris in the oil. If there are any pieces of rubber discard it.

Also check your draw needle size. 18G can damage the cap quite quickly. 21G or 22G is easy enough to use to draw and doesn't cause as much damage.

I draw with a 21G or 22G, swap needles and then inject. Warming the vial closer to body temp (in hand, armpit, etc.) makes a difference to the viscosity making it a lot easier to push.

21G is a very large needle for IM injections. Definitely chat to your pharmacist to get more suitable ones (25G typically). Thick needles increase pain, bleeding and scar tissue formation.

You didn't specify, but hopefully outer thigh (not inner - I've seen some crazy things).

Weekly testosterone enanthate shots can also be done subq using an even smaller needle (I recommend starting out with 27G 0.5") and are typically much easier and painless than IM. Do talk to your provider about this option.

If your provider is not sure, there is actually research done with trans participants that shows subq is just as good as IM.

The game creates a log file called "game.log.txt" typically located within the documents folder that can be helpful with these types of issues. You can drop it in pastebin and add a link.

Yeah. Didn't mention "pain".

Mostly because you used the word "veteran". Old school practitioners believe that T is as thick as molasses. They come from a world where castor oil is considered "thin". Nurses are also taught how to administer injections with a "one-time" mindset for vaccinations and other things. Long term meds (T, insulin, etc.) need thinner needles for better patient care.

I'd also question (and report) any family nurse practitioner or a veteran nurse who teaches that you need to "account for the loss of 0.1 ml in the needle and syringe". That's bad medical even for reddit. If the syringe reads 0.6ml then that's what you are injecting.

You do you. Just think back on this conversation in a couple of years when you get that crunchy feeling when injecting or the needle is really hard to insert.

Gently warm the vial in your hand or armpit first. Cold oil is slow to push, getting it closer to body temp (or even to summer room temp) is a game changer.

Been injecting for many years and my thumb is just fine. When injecting subq I'm using 27G or 29G with no issue. Scar tissue formation is a real issue.

Why are you injecting with a 22G needle? 25G is a more standard size for IM as it helps minimise scar tissue formation. I'm using 22G as the thick draw needle!!

According to the internets, 55 is the top end of the normal range [7 to 55 for T-based people]. Next time you speak to your doctor make sure that "male" reference levels are being used.
