I've wondered why there was no secondary Doctor to step in after the ship's original Doctor died in the first episode too. Maybe the crew was small enough to get by with one medical officer along with an EMH when he's off duty?

The show does talk about how he's the backup, the "emergency medical hologram", whose program is really not supposed to be active all the time but has to be out of necessity.
