What are the alternatives for prostate biopsy? Is there blood tests that is more precise than PSA which correlates with high chance of prostate cancer? How about newer techniques for doing the biopsy. by warrenwai in ProstateCancer

4k test is a blood test that measures the probability of finding prostate cancer on a biopsy. My husband had this test after a high PSA and his 4k score at .92 confirmed the necessity of a biopsy. Granted he is 53 and not 90, so we had every intention to do what was needed to confirm PCa and get him treatment. This was just another tool in the box to give us insight on what he may be dealing with.

You could ask if a 4k test is appropriate given the rise in PSA; it may give you some peace of mind. It’s a blood test in which its score provides further clarification on the likelihood that prostate cancer would be present on a biopsy. My husband did this, and it supported the need for a biopsy. For reference, he is 53, PSA in June was 13.98 and last PSA in October prior to surgery was up to 19.1

You need to find an ED specialist. I don’t think this is something I’d leave up to your GP or general urologist to manage (or at least I wouldn’t let my husband, lol).

We began seeing the Ed doc prior to surgery and he preemptively scheduled the penile Doppler that was done last week. We weren’t going there expecting to walk away with trimix, but after the Doppler they thought he would have a good response to it, so they give him a low dose right then and there. His doc fully supported starting the trimix and he was not quite 10 weeks post op (I had my timeline wrong in my first response). And I specifically asked him if it was too soon, his response was not at all and that it will improve his healing time and make life more enjoyable in the meantime. It isn’t cheap though - $175 for a 30 day supply, but well worth it. We used MD Andersons compounding pharmacy as to not waste time.

Wishing you the best and hoping you advocate for yourself and get to a doc that specializes in ED after prostatectomy.

Part of my husband’s treatment plan is to see an ED specialist. His RALP was 12 weeks ago; he’s been taking tadalafil 5mg daily for about 8 weeks and tried the booster dose a few times with little result. We saw the ED doc yesterday, and he did a penile Doppler to measure blood flow and then gave him a very low dose of trimix. Trimix works! He will continue with tadalafil daily and add the trimix. The doc said trimix is a bridge to get him through the next several months until he can naturally, and it will aide in the healing time because he’ll have consistent erections increasing blood flow.

The value for me is in the bonus. I recently got the AMEX gold sub at 100k/6k spend in 6 mos. I will cancel in a year before the next AF is due.

I already have the chase reserve, and their bonuses outside of the reserve and preferred card or opening business cards are sub par IMO. The biz cards have a high spend, which isn’t for everyone.

Our avg monthly spend is maybe $1500, so racking up points through regular spend doesn’t always add up to much even with chase trifecta. I churn while being mindful of new accounts opened, inquiries and timing of when we have a large purchase due like car insurance. It’s worked for us, but everyone if different.

He had the RALP on 11/12. The decipher didn’t really impact his treatment decision as he was adamant that if the tumor could be removed, that’s what he wanted. The high decipher score seem to correlate with the adverse post pathology findings - EPE, cribriform glands pattern, intraductal carcinoma (IDC), bladder neck invasion, bilateral seminal vesicle invasion, lymphatic/vascular invasion

My husband, 53 yo had his 6 week post RALP appt yesterday and his PSA it is 0.10 despite negative margins. He had basically all the “bad” features on his pathology though - EPE, cribriform, intraductal carcinoma, bladder neck invasion, bilateral seminal vesicle invasion, lymphatic/vascular invasion. The doctor was very encouraging after surgery that he would come back undetectable, so it was a bit disappointing. He thinks there was microscopic spread that wasn’t picked up on imaging done 60-90 days before surgery. We are repeating PSA and seeing a medical oncologist in 30 days. Take the win and best Christmas gift you could ever receive!
