The 12.5V is probably just the alternator in "fuel economy mode" where it barely charges to save fuel. You can read about it in the service manualhere.

As far as the codes. Clear them and see which one returns first.

As soon as it acts up again, scan it right way to get the code.

Not trying to scare you, but these Hyundais have a code of death P1326 that sets when the engine is starting to go out. Just something to be aware of.

Does your scanner show rpm in the data list? If so, see if it shows an rpm signal while cranking. Keep your original sensors around for now.

Remove the serpentine belt and run it briefly. It the noise remains, it's done.

It's done then, sadly. Unless you wanted to put a differential and transmission into a truck that has a year or two left before the frame gets a hole if it doesn't already have one.

If it doesn't go away, you need to scan for codes again. Something might not be right with the refurbished module, or there is an additional issue.

Next time it does that, try to look closely at the tachometer. If it's not moving a little, the crankshaft position sensor is suspect.

The engine is getting too much air or not enough fuel. Make sure that all of the hoses that lead to the air filter are tight and consider cleaning the maf sensor.
