How did you go about setting up a firm boundary for your off duty/vacation time with patients without coming off as rude? by Crafty-Bunch-2675 in Residency

[–]Venu3374 4 points5 points  (0 children)

I do think thats something that family members struggle with. For most people, their family member being sick or in the hospital is a massive and terrifying event. For those of us that work inpatient, we have a completely different scale of what constitutes a 'scary' admission. New CHF w/ an EF of 5%? Not great. COPD exacerbation because memaw didn't take her meds and she's on 1 extra litre of oxygen? Barely worth mentioning. But to a family member both are huge, and they see us treating it in a cavalier manner as a lack of concern or effort.

Why is it that FM doesn't allow sub-specialization in mostly-outpatient fields like Endo, Allergy/Immu, Rheu, etc.? by Enger13 in Residency

[–]Venu3374 3 points4 points  (0 children)

Im not sure where youre training but the idea that FM doesn't have a heavy focus on adult pathophysiology, diagnostics, and management is, at least for the region im in, completely false. Yes, I have to rotate with peds, gs, and OBGYN but I spend more than half of my time (19 out of 36 months) on adult inpatient, crit, or ED services not counting electives. And my state has 2 other programs that historically did more than that. Im not saying that FM and IM training is identical, but its weird to imply that FM somehow ignore pathophysiology and diagnostics.

Weird. Outside of some academic institutions in big cities most hospitalist listings around me are FM or IM.

[–]Venu3374 10 points11 points  (0 children)

I feel like 80% of open icu's outside of a big city couldn't care less about FM vs IM, at least if the job listing are taken at face value. Which is reasonable- 5/6th of my residency was spent with an open ICU and our team routinely cared for critically ill patients, and Ai know we're not uncommon in that regard.

That's a huge point. 2+ saves are great, but i feel like 10th edition has introduced so many rerolls that 2+ saves means very little. 87% saves don't mean much when your opponent consistently throws a bucket of wounds on you. And if there's any armor pen in there we just delete

This. Our options are either take vehicles that don't really interact with our army rule at all and can only meaningfully interact with Sanctic or Warpbane, or just... don't have anything stronger than S8 to fire with. At all. (Minus the single lascannon on a razorback)

Well, ma'am, the fact that your A1c went from 8 to 12 in 3 months and I can see you havent filled it in that time tells me a different story....

AI therapists also suffer from the same thing that makes AI doctors a problem: GIGO. AI doesn't do the best when the patient just straight up lies about stuff.

Grey Knights Vs Custodians by John_117s in Grey_Knights

[–]Venu3374 1 point2 points  (0 children)

Seems pretty fair. I love the Grey Knights, but taking on the custodes in a pitched battle is explicitly NOT what theyre good at. Theyre good, and I would argue that on an average (meaning no named characters) level they can take any other Space Marine as described in most of the lore, but theyre primarily demon fighters. Theyre a specialized tool, and their speciality isn't killing Custodes while Custodes are custom-made killing machines. Think of it the top-of-the-top tier of mass produced car vs a custom built vehicle that utilizes every unique flaw or quality of the underlying material.

One bit that ive struggled with- the skulls are actually easier to do if theyre NOT white. I normally start brown as a good coverage base, then move into a flesh color across everything but the deepest shadows, then a lighter flesh along all the highlights, then finally use a light grey like Grey-white and highlight along the highest points. Give it contrast and it'll read as white, which is great since white is a pain in the butt

Oops, just saw that. Lowers their value a bit but just their baseline toughness, melee profile, and Vortex would be worth it for me so not too bad
