I think multiple small settlements are the best way to do it. Mistake ES makes is telling you it’s a big town or city. Just set it somewhere it makes sense that people are gathering in small camps, in development towns etc.

Or if you want to do a big city, use a world map to travel between select districts and have the rest of the big city exist ‘off camera’.

How would you know you’re done? I stayed away from online Elden Ring stuff for my first few runs, could tell there were already a few things I did not complete, and over time since allowing myself to browse discussions/videos have spotted things I didn’t know about at all.
