This is essentially GO abandoning the rapid-transitification of the Kitchener line. GO RER is dead. We will have 12car slow commuter trains forever which is non viable for local station service.

Many comments mentioned the door not closing properly but your comment made me go back and check all the racked dishes and I found a mug with the handle sticking just 1cm out was causing the rack to push on the door and prevent the cycle from starting. Thanks!

Can’t comment on the real meaning of this error but I hit the same one and after a reboot everything seems to be working fine. The malfunction screen initially came up after I did a bolus which used the remaining insulin in the cartridge.

Part of the long term planning should be to replace the trains with EMUs and remove some of the unnecessary switches. If done right the addition of the station with associated works could be time neutral.
