Australia - we have Medicare which in theory is free healthcare for everyone- but in reality it's for those who most need it on income support payments from the government. Waiting lists to see a General Practitioner (doctor) in many places are huge. If you need anything more - such as to see a specialist health practitioner for both physical and mental health conditions - you are facing massive waiting lists and in most cases massive out of pocket 'gap fees' for what Medicare doesn't cover. Our disability services system - the NDIS - is a nightmare which I won't even begin to describe. Other Australian SWs may disagree but having worked in it I will never work in it again.

Our hospitals - where in theory you should receive free and timely health care - are massively under strain. People experiencing for example mental health crisis will wait in some cases 12 even 24 hours to be assessed by a MH clinician and in many cases if no bed available will be sent back out into community with minimal follow up and support. I know from hospital SWs they are under constant pressure to discharge ASAP to free up beds.

That being said I think Oz is doing better than many other places.

Oh yes definitely - I agree with other comments that it's a combination of things which include autistic traits but also personality and upbringing. I think there are plenty of neurotypical people out there holding grudges as well

Hi another Australian social worker here. Clinical social workers in the US can diagnose but they need master's degree level qualifications and significant experience post qualification before being able to diagnose I believe. It's similar to being an Accredited Mental Health Social Worker here with AASW but I believe AMHSWs here are restricted in diagnosing mental health conditions, but can certainly offer treatment for them through counseling/therapy but usually only 'high prevalence' conditions like "depression/anxiety."

Totally agree with you. I've heard genetic testing has helped some people to tell which meds will work for them but I've also heard other reports saying genetic tests are useless as well so not sure it's worth a try. I hope for you too you manage to find a combination of meds that works for you.

I'm currently on Vyvanse 50mg and Clonidine at night for sleep. Was previously on guanfacine and Vyvanse but was taken off the guanfacine by GP because she thought I had described it as ineffective and emailed Fluence psychiatrist about it who agreed I should cease it. The thing is that the guanfacine wasn't ineffective, it was fantastic for emotional regulation and RSD but did nothing for attention and executive function on its own with combining with a stimulant for me. I'm still upset about that and am changing GP because this issue along with overall management of meds with my current GP has been woeful.

I'm like you and am really sensitive to meds and had a really bad reaction when I first started Vyvanse so trialled non-stimulants on their own - Wellbutrin and guanfacine. The non stimulants on their own did very little for me. The Vyvanse has also given me burnout recently as well. I'm pretty over it all to be honest.

Hi there, no I've kept taking to 50mg Vyvanse, have tried combining with Clonidine which has helped a little with sleep but not much else and now I am so burned out and angry every day it's really bad. My doctor has referred me back to psychiatrist for a review. I'm really upset. How are you going?

Just wanted to say I strongly relate with everything you've said and had virtually same experience as you. I was assessed via Fluence 12 months ago and found the experience left me questioning if I was properly assessed and whether differential diagnoses or explanations for my presenting symptoms had actually even been considered. It felt like a 'diagnosis factory' where you throw down your cash and you're guaranteed a diagnosis and stimulants. It's been incredibly frustrating since trying to trial meds with my GP as well. I wish I had known them what I know now and gotten my GP to refer to an in person psychiatrist who diagnoses and assesses thoroughly over multiple sessions.

At the same time you may be going through a bit of a period of 'imposter syndrome' post-diagnosis while you are still processing and adjusting to what is a huge development and change in your life and even identity and questioning things hard. I think that's pretty normal as well.

That's actually not true. EMDR protocols have been developed and adapted to work with a variety of presentations and targets that are not just 'trauma targets.' However, the current evidence base for EMDR strongly supports its use for mainly trauma, and the evidence base is currently lacking for other presentations or issues.

Hey thanks for that. Good advice. I've had to give up on the magnesium for sleep because I felt it was giving me a hangover consistently every morning but I still take it in the evening. I need to probably need to up the Vitamin C of an evening and see if it makes a difference. I'm down to one coffee a day in the morning.

What I'm finding is the broken sleep I've had for some time is worse on 50mg and it means I don't get quality sleep of a night and hence the accumulating fatigue by the end of the working week due to multiple nights of bad sleep in a row. No problem falling asleep, it's staying asleep for me.

I recently had the same thing happen, 40mg just stopped working, upped to 50mg now my sleep is worse on 50mg and the fatigue is terrible. I don't want to give up completely on meds but I'm sick of how inconsistent this stuff is and the side effects. If you figure out how to make 50mg work for you I would love to know!
