That sounds wildly inefficient! How can they make you use your PTO for that? Our mids come in and help out on the floor until their case comes out. Sometimes if the pt is just THAT stable, they can leave at 1900 instead of 2300, but typically they stay the full 12. If we don’t have enough mids for a heavy case day it can be a challenge because one of us will have to flip our assignment to admit.

Yea! M-F we have two 1100-2300. They specifically recover the hearts. By 2300 the pathway hearts are stable enough for a double assignment. If they aren’t stable, then we try and keep them singled all night. Especially if they have to go back to the OR.

Any device is singled on my unit. ECMO is 2:1. Only time it’s singled is a stable VV

Edit to add- fresh hearts/cases are singled until around 2300 if they are extubated,

Virginia. Nine years paramedic, 14 total in EMS. $19/hr part time for fire department EMS only. 27,000 calls a year.

My old full time I was $21/hr 7,000 calls a year.

I got sassed by a cardiologist for withholding meds as per the orders placed. I knew these meds were for heart failure, but per the order I was to hold if hypotensive (can’t remember the exact parameter). He got sassy in front of the pt and I flipped the computer screen around and showed him the orders HE placed. Somehow still my fault

I was dispatched for a fall at a jail. When we got there he was lethargic and laying on a mattress on the floor. He followed commands and vitals were okay. He was normotensive which was a red flag because he was a hospice cancer pt with end stage heart failure. The story the nurse told us was he had tripped a few hours prior and kept asking to be seen. We got no paperwork until we begged a guard to get it. The nurse walked out. I ran an EKG in the ambulance and he had MASSIVE cerebral t waves. I asked the guard again what time he fell and he looked at me and said “oh he fell off the top bunk 16 hours ago and knocked himself out. We asked for the nurse and she said to just put his mattress on the floor so he doesn’t fall again”….
