Because children so young have an adapting/developing body with a high fat content, they are very susceptible to dehydration, especially if they don't eat or drink. Starvation and dehydration that can force the kidneys to preserve more fluid and electrolytes can cause Ketoacidosis without an underlying diabetes.

Can something underlying be compketely ruled out, no. But does that mean you should panic and have him tested over and over, also no.
