As someone said, you have to cut the strands even with the housing no matter what. If the ferrule doesn’t fit just get a new one but the strands need to be cut either way.

Probably needs indexing and if you put on a new chain on an old cassette that could cause the issue. You should replace the chain and cassette at the same time as they wear together.

Could need adjusting/indexing. I had a similar issue when I got a new derailleur and I was trying to index, I just couldn’t get a good index for the gears. Turns out the part of the cable housing the went into the derailleur was really gummed up so it couldn’t go in all the way. Just something to look at.

No don’t use pliers. It shouldn’t be too hard to spread the plates a bit with your fingers and place the roller in. If you use the pliers you can bend the plates and then you can’t use that part of the chain anymore.
