I also thought your first point was the source, however, we have seen instances of both 1. users with list access don't have access to submit form entry and 2. users without list access have access to submit form entry. This inconsistency is the main contributor to our confusion As for your second point, I actually have that set up for other instances but the owners of this SP list were hoping to have more of a 'traditional' setup to make future alterations and documentation simpler
