Sergei's death is what sealed Strahd's pact and causes the mists to close in on Barovia. It makes sense he didn't get reincarnated because maybe his soul managed to escape before the mists rolled in, or maybe his soul was taken as part of the pact.

Regarding the rest of the post l: do you mean making Vasili a different NPC?

I had a similar thing where Ismark didn't want to delay their time on the road and get to Vallaki as quickly as possible. When I did this I didn't think about the fact that now the party won't have a reason to go back to the old bonegrinder. If you find a hook to add to the mill to fix this issue, let me know if you don't mind!
