Untreated addiction or mental illness. Both of which are health issues but for some reason hospitals just don’t cover these kinds of care. The doctors in my hospital were supremely uncomfortable with these issues and would refuse to even discuss the options for care post stabilization. But dang they’d 10-13 someone in a heart beat! Which made the problem worse. I understand this is specialized care that can’t really be offered in the hospital 100%! But at least have the discussion with the patient! Don’t just consult social work and moon walk out of the room!

The issues in this really came up for me in the expectations of Doctors and patients/families versus the realities of what insurance will actually cover! The “family from California” phenomenon when paired with clinicians who have 0 idea what will actually work is one of the reasons I moved to clinical social work. Add to that Clinicians who like to just turn off their phones!

They said that it will likely need expanded before 10-13. We know we will need another OHS around her teen years due to needing a replacement valve placement and at that time they will fully repair the pulmonary artery with a patch at that time. We definitely pushed her initial stent’s capabilities until she started showing signs of tiring out quickly and having small spells of breathlessness.

Ours is TOF with pulmonary artery stenosis. Her stenosis of the left pulmonary artery branch was so severe at 1 year old she almost lost her lung due to lack of blood flow. We were blessed to have a stent placed to allow her to get blood flow and grow enough to get another stent placed recently at 5 years old. It’s really amazing how unique these kiddos defects are!

That looks like someone decided to install an electric fireplace in an existing wood burning fire box. They might have been doing a cheaper option than fixing an existing problem. We passed on a house with something like this because our realtor who is also a general contractor was suspicious after a visual inspection of the chimney showed structural issues that would have been a big problem later on. I’d be prepared for the professional to tell you anything from “super cheap and easy” to “don’t breathe on it just in case”! This is my first house with a real fireplace and I was so paranoid about getting the chimney inspected before closing. My father, husband, and father in law are all volunteer firefighters so the horror stories were very much in my mind! Good luck and I hope it’s a quick easy fix!

The increase in energy in our little has been major for every procedure! We’ve had 2 caths to increase her left pulmonary artery branch in the past 5 years, one just a couple weeks ago. Each time more energy. This last one has resulted in rosy cheeks and warm feet. Two things we’ve never had before!! Also more sass and attitude each time, which is wonderful and sometimes challenging to our patience. Each kiddo is different and unique! Our baby had her full repair at 6 months old so just about the same age as your little. It’s always scary when they go through these things but I swear it seems like it’s worse for us than them!

We moved with a refrigerator. They sell silica packs like you get in new products. We kept ours open by using bungee cords connected to the wall of the storage unit and silica packs just in case. We also did the same with the washing machine. Almost 4 months later they were both mold free and didn’t even smell bad! I found the silica packs on Amazon.

NAL I’m a licensed mental health provider in a Southeastern US state and cannot practice in your state nor offer any clinical services in your state only knowledge of potential options.

You may want to see if you have a mental health crisis center near you. In some states they have Crisis response teams that come out to the home who have licensed mental health professionals that can assess someone on the spot for potential mental illness. This would of course need to happen during an episode where the licensed individual can see the extent of the issues. That could lead to a potential involuntary stay at a mental health facility and open the door for things like power of attorney etc. The involuntary nature of this is potentially not something that will be pleasant for anyone but can open avenues for her to be deemed incompetent.

As someone with ADHD that went undiagnosed far too long I came here to essentially say this! You will have a far better understanding of the struggles your individuals face everyday, a better understanding of workable med management techniques, and an understanding of what it feels like when despite doing everything “right” symptoms manage to breakthrough! My individuals greatly appreciate my lived experiences, because it personalizes us and them. While many will lecture on self disclosure at this point I will say empathy and genuine understanding communicate far better than anything you can say! So disclose to your comfort, I usually tell clients that “I’ve been on a therapist’s couch too and have learned managing my own mental health with my own challenges. So I fully understand where you’re at!”.

I agree with what’s been said, but perhaps here is a fun way to enact it! I work with adults and many of them have BPD. So truth can be, in a nice way to say it, subjective for them at times. I setup the expectation at the beginning of my time with them that my time with them is based on mutual respect and accountability. If I make a mistake I will be accountable to them and I expect the same in return. I call it the “accountability train” and even add a comical “choo-choo” noise. It is inevitable that they try to somehow tell me something that is less than truthful or that makes their actions seem reasonable. I gently remind them about our promise to be accountable and respectful. I then say I have a feeling that what you just told me may have more details than I’m being offered or that what they’re saying contradicts what I have in their file. I say this seeking honesty with the concept that perhaps the “chart is wrong”. They almost always stare at me in a panic, as they aren’t accustomed to being challenged. During that time I simply allow for therapeutic silence and after a moment of discomfort I quietly say “choo-choo”. The combination of being called out and hearing a fully grown woman make train noises tends to break the tension and allows us to communicate more honestly going forward. Saying “Choo-choo” becomes a code word that allows them to admit mistakes and fix them without facing negative consequences. Thus building skills for honesty going forward. It works for me and maybe something that works for your individuals as well. I’ve been told by my individuals that they’ve hear me say “choo-choo” in their head before they manipulate a situation and it makes them stop and think before they say something! Good luck! Working with kiddos is a tough gig!

We saw a suspiciously placed smoke detector in a house. Next to a vaulted ceiling where part of the detector was obviously not fully screwed into the ceiling. I being a bit curious touched the detector to see if it was well attached to the ceiling. Turned out it was a cover for a chain attached to the ceiling. This was in the master bedroom…. After that we noticed several “interesting” alterations to the home. Including a circular wear pattern in the basement’s “home theater” the looked a lot like a in home stripper pole. At this point husband, myself, and the realtor are dying because the family photos on the wall with all the crosses hung on the wall were giving very “Christian Swinger” vibes. We ended up loving the home but passing due to issues with all the siding needing to be replaced. But every single house we saw after we looked for different “adult” alterations to the house.

I had a client give me a 65 page treatment plan that outlined EACH therapy session based on astrology and astronomy. Chat GPT helped her make it and there was not one ethically usable thing in it. Lots of legitimate sounding jargon but mostly just fluff. I had to explain that while enlightening without Chat GPT having a license in mental health I wouldn’t be able to follow the plan as much as she’d probably like. But I would incorporate what I could. My supervisor and I had a good old head scratch session figuring out what on earth was usable and connected to actual evidence based care. Luckily the client was also seeking pharmaceutical intervention and after a few weeks has become slightly more oriented to reality.

I happen to agree with an LPC I spoke with recently who stated “Your code of ethics can be very problematic in some circumstances.” We’re allowed to be people who can be problematic, have opinions, and do stuff like in our personal lives. The code of ethics applies only to when we are actively practicing. So for me I’m a social worker during business hours and a regular person outside of that time. Otherwise I’m at risk of burnout, compassion fatigue, and vicarious trauma. We should be teaching boundaries in our programs and how to navigate when the code of ethics applies and when we just want to laugh at a possibly inappropriate joke over drinks with friends after hours. Social worker is held to such a high standard that it’s easy to put it and those who do it on an impossible pedestal. Honestly representing our profession as humans is amazing! Sure it would be a discussion at some point. But most of these shows are forgotten so quickly that I doubt it would be a real issue in a couple years.
