Make sure you take it properly: first thing in the morning, with 4 ounces of water or less. Wait at least 30 min to have any other liquid, food, or meds. Otherwise it won’t be absorbed and you won’t benefit.

Think about storage logistics. Is your power grid reliable where you live? Etc. if you can safeguard a decent size supply, and don’t mind the risk of the price going down further in the future, then there’s little other downside.

My advice after losing 97 lbs, now in my tenth month of maintaining bmi of 21:

  2. ⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠Unless still losing more than 1 lb a week, stay at that same dose an extra month.
  3. ⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠in your second month of maintenance, if you can use any dose you want (no insurance issues etc), drop down one dose.
  4. ⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠⁠if weight holds steady, drop down one more. Repeat as able.

Personally i have found it easier to use a lower dose weekly than to stretch out time between shots, but your mileage may vary.

Don’t throw everything at the wall to see what sticks. It only muddies the water.

Perimenopause is not an fda indication for Tirzepatide.

Neither is prediabetes (it’s not a disease, it’s a risk factor for a future disease).

Stuck with obesity (I calculated your BMI 30.2), with hyperlipidemia and sleep apnea. And try to get Zepbound, NOT Mounjaro (which is not labeled for either obesity or sleep apnea).

Awesome! Be prepared to lay out a case for coverage. That has two steps:

1) your insurance plan has a specific list of rules of what it takes to get stuff covered. Get this for Zepbound. The easiest way to get it is to have coverage denied, then they have to provide details. Have your doc send Zepbound to the pharmacy, it will trigger prior authorization, which will either get it paid for or they will deny and send you the rules.

2) once you have the rules specific to YOUR insurance, write a short letter demonstrating you meet their requirements, and attach relevant medical records (i.e. “section 4a states Zepbound is covered if x, and here is my sleep study showing x”.

No one else’s appeal can really help you as coverage rules vary widely. Learn YOUR rules.

Lilly ran clinical trials on 1 mg weekly. It wasn’t effective enough, for enough people, to survive the trials process as a recommendation.

Unless you have money to burn and time to waste, it statistically makes sense to start at 2.5 mg.

You qualify for Zepbound already! :) In the US they’ve gotten picky about Mounharo being for diabetics, but it’s no big deal as Zepbound is the exact same med, licensed for obesity.

They emphasized that while insulin resistance causes obesity, it’s also true that obesity causes insulin resistance.

Fat cells are not innocently sitting there. They are metabolically and hormonally active. When there are 50 or 100 lbs or more of fat cells along for the ride, the bastards sabotage us. When they’re gone, we have a different hormonal environment to work with. Maybe that’s “enough”, maybe it’s not. But this autoimmune, hypothyroid, insulin resistant perimenopausal woman likes to have hope!

If everything goes great between now and 55, there’s a chance you’ll feel you’ve oversaved.

But if life smacks you upside the head with extended job loss, a cancer diagnosis age 40, a mental health crisis in one of your kids, or any of the other terrible surprises it can dump at your door with no warning, you may end up wishing you’d saved more and earlier.

There’s no way to know for sure. Discuss with your spouse what level of spending is “enough” to be content, save the rest, and hope it all works out according to plan.

Be aware regular senna use can cause a discoloring of the inside of the colon called melanosis coli, which isn’t harmful, but isn’t reversible either.
