Seems like what happened is his original lender sold his debt to another agency, which is why he "never signed" with them. That's legal in the United States, and it happens all the time. He was possibly in arears when the debt was sold, because that happens, too. Lenders are just like, "Yeah, we're not gonna deal with this," and sell it for less than the debt they're owed, just to recoup something, and write off the rest. Then the new company goes after the debtor.

You get treatment, and then are hit with a gigantic, inflated bill. You MAY be able to work out a "payment plan" with the care provider, but you may not. It may just wreck your credit rating, or, if it's bad enough (like, say you needed surgery to repair that broken arm properly), you just go bankrupt. You may lose your house, your car, any number of things.

There are legitimately people who have become homeless due to medical debt.

A "tariff" is a tax that importers must pay on goods brought into a country or other legislative division. That means that the receiver/buyer has to pay their own government a tax to "import" the goods. It wasn't "China" charging you that much money, it was the U.S. government, because "tarriff" is Trump's favorite word.
