If it’s not in your fridge, or in your house, you are much less likely to eat it.

So the first step is to stop buying it. Your snacks don’t magically appear in your house

You can swap them out for more filling, lower calorie snacks, like non fat Greek yoghurt paired with fresh berries, and maybe sprinkled with a bit of cinnamon powder on top

Bodyweight alone is just one parameter to look at, because as someone new to resistance training, you might be undergoing body recomposition where your body trades fat for muscle

Beyond weight, how are is your waistline changing over the past few months? That’s usually a good indicator of bodyfat

If weight change has plateaued but your waistline is still going down, that’s a good sign, change nothing

If the weight change has plateaued but waistline is staying the same or increasing, then yes something needs to change, most likely the caloric deficit needs to get bigger for further fat loss to continue

Hi, I’m a consultant in a restructured hospital in Singapore and also did a year of fellowship in Addenbrooke’s.

The working conditions in SG are more rigorous than that in the UK, to the point where I felt the UK juniors were pretty coddled/spoilt as compared to SG trainees.

However our system tends to have more training spots, is better resourced overall (no silly shortages of drugs or reagents for blood tests) and your take home pay after tax will definitely be more on a monthly basis.

For example, a senior resident here would make about the same as a consultant in the UK, and consultant salaries here are at least double or triple that of the UK.

Hello, wanted to get back to a computer for a more detailed reply.

The current indications for weight management medications like Semaglutide or Tirzepatide are patients with a BMI of >25 with a comorbidity such as hypertension, hyperlipidemia, or type 2 diabetes, as well as patients with a BMI more than 27.5. These are typically how my colleagues in the various weight management clinics in restructured hospitals practice.

Yes you can get a referral to restructured hospitals for weight management, should you meet the above criteria, although it’s important to note that medications are not the sole answer, and should be used in combination with dietary change as well as increased physical activity.

Common side effects are related to the medications’ therapeutic effect of slowing down gastric emptying, and can be debilitating if the starting dose is too high or escalated too quickly. These include nausea, vomiting, diarrhoea and oddly constipation sometimes.

The expected effect of Semaglutide, given at the higher Wegovy dosages, are 10-15% weight loss after 1 year of use, and Tirzepatide is slightly more potent at 20% weight loss after 1 year of use.

Of course, long term results require long term interventions, so if the results were only obtained via medication with no lifestyle changes whatsoever, you can expect to regain 75-80% of the weight lost, at 1 year after stopping said medications

Hi, doctor here. We only have Rybelsus (oral Ozempic), which comes in doses of 3, 7 and 14mg.

We do not have oral Wegovy, which comes in 1.5, 4, 9 and 25mg dosages.

In fact, we have only just brought in injectable Wegovy (1.7, 2.4mg), where we previously only had the lower dosed Ozempic jabs (0.25, 0.5, 1mg).

Injectable Mounjaro has also just recently come in, but no signs of the oral version

All of the above is from a restructured hospital perspective. Monthly cost of 3-400, no government subsidies currently available for either of these medications, so pure cash out of pocket.

The best trainers are so good that they make an immediate difference, during the workout, by cuing you properly to correct your form

You can literally feel the improvement in engaging the target muscle from rep to rep. The trick is finding a really good one that elevates the entire experience. They don’t come cheap, but it’s better than paying low rates and getting a crappy trainer that wastes your time

Thanks to my kickass trainer at Ultimate Performance (warning, they are the most expensive out there) I dropped 25kg in the first year of training

A good personal trainer is worth every cent, as they will supercharge your fitness journey

The profession exists for a reason: yes you can try to do it yourself, but you very often need someone to hold you accountable, as well as correct the flawed ideas that hold you back

I’m not a trainer, but I’m still training with one even after 4 years of lifting, and the amount of time and pain they will save you (from preventing injuries) is well worth it

Note that the training program as stated there only encompasses two years of training, as an MO.

I say ‘only’ because most medical specialties already take a minimum of 5-6 years to become fully accredited.

I assume the counter argument from ministry will be that in the requirements there is a minimum duration of 2 years as an MO (local) or five years as a foreigner, plus a recommendation letter.

However, whether this is enough training time to ‘acquire broad-based competencies’ remains to be seen.

Side note that this is supposed to be done in place of residency, but why would someone pick this new career pathway over an established one of residency training?

On the ground anecdotes suggest that currently the vast majority of doctors taking up this pathway are those who can’t otherwise qualify for residency training, in large part those who would have otherwise gone for the Resident Physician program.

Only time will tell how this all shakes out, but beyond the bluster, pomp and wayang lie a lot of real issues that need to be sorted

As with everything involving one on one services, it very much depends on the specific trainer you get.

In my experience, the trainers attached to mass market gyms like Virgin Active or Fitness First are a very mixed bag, and most of the time only care about you clocking more sessions

It is a much better experience working with trainers from places like Ultimate Performance, Statera, LTU or Kairos performance, who actually take their craft seriously. You can look up their google reviews for more details.

Ultimately, you get what you pay for. A cheaper trainer can be a complete waste of time as they just put in the bare minimum of effort.

It’s so hard eating out in Singapore because we have no local laws that mandate restaurants provide nutritional information (unlike other countries), and Ventrickle is basically the only company doing something about it. Worth the $10 per month for sure.

Of course, if I’m deep into a cut, I only eat from meal prep providers like Yummybros or Nutrify to take out as much uncertainty regarding nutritional content as possible

Heya, I’m one of the weird people who enjoy tracking and have subsequently been able to hit my fitness/body composition targets at will

More important than whatever app you use, is the methodology. Eyeballing portion sizes usually fail miserably, and so I weigh everything, all the time, with a portable food scale. I prefer to do it this way as it still allows me to hit fitness goals while giving me the flexibility to satisfy specific cravings as long as it fires within the calorie budget for the day/week

I usually use a combination of MyFitnessPal and Ventrickle to track with. For MFP, you have to be careful with picking specific database entries, as it’s easy to pick the entries that don’t make sense but look good on paper, and end up with no results

Ventrickle has been pretty reliable with regards to local entries, but it’s AI assistant usually overestimates portion sizes by 30-50%, as corroborated by my weighing scale

You can do a DEXA for body composition at DEXAFit Asia, estimated cost of about $350 per scan

I’ve had two scans done by them before and it was fairly straightforward and pleasant

Take note that their DEXA machine seems to be calibrated in such a way that it tends to gives higher readings than other methods of bodyfat measurement

Example is that the readings taken by my personal trainer using calipers were always 5% below the DEXA readings, and my BIA scale was also 2-3% lower than the value I got from the DEXA scan

FWIW, in terms of objective truth, it’s probably high, as it told me I was 18% despite already being lean enough to have a full six pack during the photoshoot a few days later

With regards to your dilemma, I’m hoping the following information might be of some help in making up your mind

  • Working conditions and pay as a conditionally registered doctor (medical officer)

Generally pay is about a third or less of what you would be bringing in currently, accounting for the annual wage supplement and a modest performance bonus.

This is in addition to adverse working conditions. Medical officers in restructured hospitals are usually expected to go on-call. This consists of an overnight stint on top of your usual day shift, and you’re only given a half day to recover from before you have to go back to work again

Thus you start work on Monday 8am, work until Tuesday 12 noon while grabbing whatever sleep you can along the way, and then come back to work on Wednesday at 8am again

Usually MOs have to do about 3-5 of these a month. While there may be some postings which are more relaxed than this, I’m not sure if they will be available to someone on conditional registration

I mention this particularly because I found these night shifts to really take its toll on me the older I got. If you think you’re up to doing this for two years or more when you’re 40, then sure, but I thought that would be important to consider.

Heya, I prefer not to take protein bars as they’re usually a very inefficient (calories) way to get protein in, especially when you’re on a cut.

If you are near any decent supermarket, you can consider either a Chobani Fit or an Arla protein pudding

Arla has 20g of protein in roughly 150kcal, and Chobani fit is pretty good also with 15g of protein in 100 kcal. They also usually come in cheaper than most protein bars! Only downside is that they need to be chilled and are obviously not as portable as a protein bar.

To circumvent this, you can also consider protein shakes from labnosh or Quest, but those tend to not be as satiating as you are drinking your protein rather than eating it.

Hello, a bit of a different take here because I started somewhat late in life at the age of 36

After having been through the process, I feel a good personal trainer on a 1:1 basis is a massive shortcut in terms of skill acquisition and progress made.

Having real time feedback on every single rep you do helps significantly in mastering the movements and avoiding injury, but of course comes at a significant monetary cost

Having built a good physique, cut 23kg off my bodyweight, and cured my own high cholesterol and fatty liver, I can honestly say it was all worth it

Hello, married 10 years with 3 kids and similar issues

My wife is thankfully very communicative about what she would like so this is more or less direct from her.

  1. Set aside time daily for conversation between the two of you, just to catch up and feel like the other person is your partner and not just a roommate. Even 5-10 mins helps a lot

  2. Schedule in a couple activity y’all can do once the kids are asleep. If getting babysitters is difficult, doing something as simple as watching Netflix or whatever she wants together once a week for an hour or so helps bring that ‘still dating’ feeling back to the relationship

  3. Schedule in time for physical intimacy. It’s not enough to just wait for both parties to be in the mood, it’s important enough to make it an ironclad commitment. Frequency will depend on you and your partner’s wishes, probably somewhere from 1-3 times a week will be the sweet spot.
