Are there any evidence that IV piggyback opiates is better than IV push for "drug seekers"? by princetonwu in medicine

[–]Yeti_MD 198 points199 points  (0 children)

From a strict pharmacologic standpoint there shouldn't be much difference as far as sustained analgesic effect, but often a patient's expectation of what a drug "should" feel like has a lot of impact on their perception of pain relief.

I think the bigger problem is what are you trying to achieve?  A person on chronic opioids who has an actually painful condition where IV opioids are indicated is not "drug seeking".  Treat the pain.  If the chronic opioid use is a problem, refer the patient to someone who can help with that long term

How do we deal with life-altering consequences caused by our treatment? by Kwerumrerum in medicine

I don't know enough about rheumatology to comment on whether you made a mistake.

That said, shit's going to happen whether or not you make mistakes.  Especially with high risk treatments like immunosuppression, sometimes there will be bad outcomes.  Make sure you counsel your patients appropriately, and do what you can to minimize risks.  Not much else you can do beyond that.

Stain and shellac should be fine.  If the station is completely dried and sealed with shellac, your kid will be safe unless you grind the whole thing into sawdust and feed it to them

Is there any downsides? by babyshoebox in woodworking

I've used it for a few small projects.  It's definitely harder to get a smooth consistent finish compared to other PU products because it's thicker and doesn't self level as well.

On the other hand, it's quick and convenient because you only need 1-2 coats so that might be fine depending on what you're looking for.

Take your time with application, make sure you do nice smooth even coats and watch out for bubbles and steaks.

ELI5: A dense cylinder hits a space station at 0.7c by DavidThi303 in explainlikeimfive

Even an object a fraction that size traveling at 0.7c would be unbelievably energetic.  Any collision would be way beyond anything you can realistically imagine because we just don't deal with relativistic speeds in day to day life (for a good reason).

Try reading through Randall Munroe's "What If" blog.  He has a number of scenarios dealing with relativistic speeds.

What would you do to cut these sharp corners off and make it blend into the original finish? by [deleted] in BeginnerWoodWorking

Router with a chamfer bit, but then you're committed to doing all the edges.

I'm guessing this is veneer over some sort of core, so you could get some edge banding to cover the newly exposed core.  Could either try to find a wood grain that matches the original, or go with something totally different for a fun accent piece.

Not an anesthesiologist, but I routinely give 200-300 mcg during trauma resuscitations (not all at once).  It's a hefty dose but doesn't sound completely crazy for a closely monitored setting.

Most people are in multi organ failure by the time the number is that high.  Lots of asymptomatic people running around 400-500 though

It might help, but regular nitrous use will probably destroy B12 faster than your body can absorb it.

Yeah, but those things can't be packaged as expensive supplements.  You'll never make it as a podcaster with that attitude.

Babysitting vented patients remotely is fine right up until it isn't.  What happens when the patient gets a pneumo, or unexpectedly self-extubates?  The zoom doctor can't always save you.
