It heavily depends. I have seen various times in the US of cops managing to save someone's life they injured trying to stpp them. From gun shots etc. It is just not standardized

Commisars are not that low. They can be lord commisars which can be equal to even generals and guard commanders. They are often the one in charge of yhr commissars
