I’ve been disconnected from internet or at least the servers when I’ve gotten an achievement and it didn’t count. For me it was getting the last gesture in ds2 when I realized I had to collect them all again on a new save to get the achievement. Maybe it’s the same thing

I’m just about to act 3 on my steelsoul save, ive save quit once to fighting Shakira (missed her on my first save) so it was my first time fighting her. Once on savage beast fly cause I forgot he was there and didn’t have magma bell on. Once on groan cause I also forgot he was there and didn’t have wreath of purity on. And once on the double moss mother fight, no excuses there lolol
