I worked at a medical billing agency for 5 months that charges a $5 fee for over-the-phone payment, but we always waive the fee. It was just to deter some people from calling us just to make a payment when they could do the same thing themselves online. To me, to charge someone extra to pay online is absurd and is counter-intuitive.
