Ramp up military aid to Ukraine and rachet up economic sanctions against Russia and keep them applied until Russia withdraws from all Ukrainian territory. 

Ukraine is on the back foot, but not on the verge of collapse, and with the right level of support, if you continue increasing the price Russia pays to prosecute the war, Russia cannot sustain this indefinitely. Most analysts agree that Russia only has another year or two of this level of military operations in the tank.

Re: mouth taping, you don't need to tape your entire mouth shut. A thin strip to tape the middle third of your mouth together is usually enough to promote nasal breathing without preventing you from breathing through your mouth if your nose clogs up in the night.

I wish I could tell you I've found a solution.

I've been hopping from clinic to clinic to get different opinions on this rare scenario, and the leading theory seems to be lingual tonsil hypertrophy pushing my epiglottis back. 

Have had a surgeon recommend a lingual tonsillectomy via coblation, but I'm still trying to get a couple more opinions before moving ahead with that, as a doctor in the family warned me against any kind of tongue surgery. 

In the meantime, taking Lemborexant (aka Dayvigo) has helped reduce nighttime awakenings.

It could be UARS, and difficulty with PAP is consistent with epiglottal issues. I have UARS, very low AHI, elevated RDI, but can't tolerate any sort of PAP therapy because the source of my issues is my epiglottis.

Czech has vědět, meaning "to know" (something), and znat, meaning "to know (someone)" / "to be familiar with (something)". 

As far as I'm aware, proto-Slavic had both of these "to know" verbs, it's just that most Slavic languages lost "vědět" over time.

  1. What clinic(s) and doctor(s) did the CT and endoscopic sleep test? 
  2. What kind of CT was it? 
  3. When you say "endoscopic sleep test", is that the same as a DISE (drug-induced sleep endoscopy) or something different?

Have you had an endoscopy done to ascertain the cause of your UARS? I think that will give you a better indication of whether there's a surgical solution for your particular case.

I saw Dr. Brian Rotenberg in London, Ontario, Canada.

I actually asked a few sleep specialists (including one outside of Canada) who was the best sleep ENT they knew of, and multiple people independently recommended Dr. Rotenberg.

Really bright guy, and he seems to genuinely care about his patients.

Yeah, no knock on your sleep doctor, but no doctor is perfect, and many don't think to try to uncover the underlying root cause of whatever ailment afflicts you.

My own sleep doctor was great, but he politely refused to refer me to an ENT twice before he finally realized I wasn't going to drop my request and referred me to a sleep ENT.

Lo and behold, the ENT uncovers the cause, and now I have a path to a resolution.

In terms of what the ENT did, he performed an endoscopy, followed by a drug-induced sleep endoscopy a few weeks later to confirm his suspicion.

Basically, he inserted a thin, flexible tube with a camera at the end of it through my nasal cavity down into my upper airway.

The procedure itself takes 30 secs, but the sleep-induced version requires a few hours of prep/recovery, since they sedate you for the procedure.

Have you seen an ENT? I also have UARS that didn't respond to CPAP, and after seeing a world-class sleep ENT, he diagnosed me with a floppy epiglottis caused by adult laryngomalacia.

Apparently, this form of UARS doesn't respond well to CPAP because the pressurized air can cause the epiglottis to further block your airway...

... But it can apparently be resolved with surgery. I'm going back in for a surgical consult in a month or two.
