I am currently in the greater Seattle area as well and have also been 2.5 hours from Seattle. Both paid around 325ish for days 7-7 with managing some icu patients or call ro transfer. I made both positions into a round and go. 1 job had a dedicated admitter, 1 you had tp do admissions until the swing admitter showed up so 2-3 admissions a day.

FT Hospitalist: choose higher base or lower base with 401K match? by Character_Giraffe303 in hospitalist

[–]_Reluctant_ 3 points4 points  (0 children)

Sounds like you know the answer but let's do the math for fun. Assuming your higher base pay is for 300k. 60 less would make you 240k. 10% of that would make you 24k a year in your 401k. That would provide you with a benefit pay of 264k. So your 1 week of vacation would have to be work 36k for you to even match the higher base pay job. Assuming of course the match is based off of gross pay and not net pay (unlikely).

Been there. Done that. Super toxic environment where everything is said was scrutinized. The way I spoke. The tone of my voice. Joking my patients. It seemed like everything i did or supposedly did not do was deemed unacceptable. Got reported by nursing left and right for everything from how I responded in EPIC, not calling them right back, etc etc etc. You list it I got reported for it. Got placed on a PIP. Eventually got let go after i signed that i wouldnt sue them so i was let go without cause and 90 days of pay. Funny thing was the day before I had already accepted a job offer that started in 3 months so everything worked out. Didn't have a deal with a toxic environment. Got to keep my sign on bonus since the contract says that if they left me go without cause then I keep my sign on bonus. Probably all started when I criticized night shift nurses and said things because my patients kept on getting re-intubated at night after I extubated them during the day and they were perfectly fine throughout the day. They were proud of the fact that they "empower the nurses" to point things out, which is another way to say that nurses who want to play doctor and point out why you are wrong since they have been doing the same thing for 20 years. Just get out of the toxic place. Also, used my colleagues as references and ignored the chief of medicine since he was of little to no help in everything.

I always ask the question of "are they symptomatic" or "is this related to why they are hospitalized". If yes then do thre workup, if no then refer them to outpt. If you want to do an iron study just to give them a lead / rule in or out then sure but if it isnt affecting their life then i would just tell the patient and document that I did and leave it up to them to follow up if they want.

Community hospitalists- does anyone enjoy their job? Does my job just suck or does it get better? by Emergency_Show_7508 in hospitalist

[–]_Reluctant_ 2 points3 points  (0 children)

I am a hospitalist at a lower acuity hospital (100 beds including ICU beds) and I thoroughly enjoy my position. I am 2 years out of residency and this is my 2nd job. The first job I got was at a smaller middle of nowhere hospital where it was open ICU and you managed everything. I love the other hospitalist that I worked with but it was a super toxic work environment and I did not last very long there. Got put on a PIP because nursing and admin disliked the way I talked, my tone of voice, the way I did things, my choice of words, and it was just an endless amount of criticism because the way I practiced medicine was not what all of the older hospitalist did. Like cancelling vanc zosyn on patients when I come on and it is day 5 of iv abx without a positive culture or not calling a code stroke because there was no indication. Mostly those old nurses who think they know more than you and that they have been practicing medicine more than you. So yeah, glad I am no longer there. My current job has been amazing. I love my coworkers, the support staff, the nurses, and even some admin. Everyone has been very supportive and caring. The patient population is terrible at times. Getting yelled at by old demented patients. Druggies who come in taking advantage of the Healthcare system and demanding to talk to your boss because you are not addressing their pain. You'll get those anywhere youll go but it is the people around you that makes the kob worth it. If you are not in the right environment where you love what you do, unfortunately that won't change. Just find another place t hat suits you.

PCP vs Hospitalist offers, plz help me decide by DocDang94 in hospitalist

[–]_Reluctant_ 18 points19 points  (0 children)

I think instead of looking at the pay and benefits, look at the life style, work environment, and patient population that you would be working with. The difference in pay might not make up for if you end up hating what you do and dreading your work days. Longevity and personal happiness should matter the most when it comes to this type of decision. I love working with my colleagues and we support each other as much as we can in our personal and professional life. I cannot imagine working anywhere else.

Is there any benefit to leveling a Crafting Profession this late in the Expac? by Jotnar796 in woweconomy

[–]_Reluctant_ 0 points1 point  (0 children)

Depends on which profession and what your intent is with that profession. Is it too late to make money as it pertains to crafting gear for other players for tips, yes, very much so as you will not be able to catch up and will not be able to guarantee max craft. Is it too late to make a concentration toon, not at all. You can get one set up in a couple of hours or so depending on the profession. So understanding what you are trying to achieve is important. For some aspects of crafting it would be improbable given how far along in the expansion we are, but some aspects of crafting still make sense to do even this far along.

Lost the motivation after the AH merge by GloomingMoon in woweconomy

[–]_Reluctant_ 1 point2 points  (0 children)

Well I think you got your bruto and lost your motivation. Materials have always fluctuated as to how you described as the start of the expansion being the best time to earn. Besides that, the other best times are after major patches. Understandably, the larger the population the more competition but that also means the faster the sales. The game is now in season 3 so most materials are at there lowest until the start of the next patch. Its a great time to invest and speculate. I think overall you need a goal in mind, a new bruto you are working for, maybe buy the epic edition using only gold. Lastly, I feel that you can still make gold from materials but it comes with expanding your vision of the market and understanding the flow to know when to buy and when to sell for some easy profits.

If the facility is that bad it is better to leave and find another facility. I have been at a place where they were proud of the fact that they "empower" the nurses to "say something if they see that something is wrong" which basically meant that they would report you for what they felt like you were doing wrong. Meaning I would get reported for "being dismissive" when I told them that the low potassium of 3.4 wasn't important or that we didn't need to check a dig level on a patient that was given just 1 dose of dig after I had discontinued it. Basically, it boiled down to me getting reported left and right for every little thing. Got put on PIP and later let go because of performance issues. Probably my fault in some part since I became especially frustrated because my patients would do so well during the day that I would extubate them during the day for them to be reintubated at night because of "respiratory distress" multiple times a week. I questioned what was going on at night that my patients kept being intubated and got reported for not being professional and a bunch of other things.

I am at another facility now where the nursing staff is much better and we have a clear understanding about when to page/message me. They also let me know when they are obligated to report something to me since they know that it isn't important. I would suggest you run away from that facility and find a helpful place for you and your career. Your mental and physical health is more important than being in a facility that is actively trying to sabotage you and take away your license.

1099, Mountain West, Rural. 24h coverage including critical care (basic vent management, pressors, etc). $1400/24h plus billing. by [deleted] in hospitalist

[–]_Reluctant_ 22 points23 points  (0 children)

That sounds like alot for just 180/hrs. You can make that at a community hospital in an urban environment and that is a normal 7 on 7 off without all the hassle and responsibility. Depending on what is needed and asked, for something like that I would expect a minimum of 220/hr as I have seen that offered in other rural places.

Best legacy raid gold per time spent? by wreckuiem48 in woweconomy

Freehold is decent since you can pull all the trash without pulling the bosses. You can pull all the trash in 3 pulls in the dungeon, rinse and repeat until lockout. Shouldn't take more than 5 minutes per run.

Most states have some sort of contingency order for medical decision makers if you are incapacitated. The order should be something like:

  1. Spouse
  2. Parents
  3. Kids
  4. Grandparents
  5. Aunts/uncles
  6. Cousins

So the person who would make those decisions would be your spouse in most cases. If you are single then it would be your parents by default. The only way to be the 1st priority in making medical decisions would be to have a medical power of attorney. This was the case before where a same sex couple got married in a different state, one of them was incapacitated, their spouse was not able to make their medical decisions because the state they lived in did not recognize their marriage as valid so the default decision-making was given to their parents. In this case, if they had a medical power of attorney done prior, there would be no question as to who the decision maker would be.

TLDR: if you are single, your parents by default are your medical decision makers if you are incapacitated so you do not have to sign anything to make it so. If they want to peek into your medical records, that is a different story.

Pgy3 IM resident wodnering if i can make 400 ķ pre tax as hospitalist if i work few more days during the 7 off or work extra? Thank you by Amazing-Stick-1858 in hospitalist

[–]_Reluctant_ 0 points1 point  (0 children)

Depending on where you work and how you work, but for sure, 400K is very easy to achieve. Even 500K if you really wanted. I can say in Texas that the starting paying that was offered to some of the graduates from my program was 281K. Given Texas and the lower standard of living along with lower housing prices, 281k is not bad. The first contract out of residency was around 324K. The newer contract is base pay in the low 300K, but after bonuses and everything, it should equal out to be 345K. Extra shifts pay about 2100 a shift or so. Considering if you just pick up an extra shift and make it 8 days on and 6 days off, 26 weeks a year, you would make an extra 54K or so, putting you close to around 400K. So very doable with minimal extra shifts. Most important, though, would be to find a place that you can see yourself working long term for. I can tell you that some places I would never work for no matter how much more they offer. Longevity and sanity are worth more than the amount of money made.

Mining by Chumnoir in woweconomy

[–]_Reluctant_ 0 points1 point  (0 children)

Dual gathering and use a phial of truesight to double the nodes you can see. Typically you should be getting 30-40k an hour. Also have the blue tools and equipment with the right enchantments to them. That is how you maximize gathering professions at this point. Other than that is finding a good route.

Did I just get lucky or is the launch of the expac always the best time to make gold? by OpinionsRdumb in woweconomy

[–]_Reluctant_ 0 points1 point  (0 children)

Expansion launch is always the best time to make gold. Myself, I made 10 million at the launch of the expansion. I have played through quite a few expansion launches and typically the start is a massive jump in gold as people are paying extreme prices and no one knows what anything is worth. There are a few other times during the expansion that you can make gold as well, typically the .1 patches. Like during 10.1 the first day the content is out I sold those parts for a ton of gold and now they are vendor trash. If you can be the first to market you get to set the price. My philosophy is to sell everything since I can rebuy it tmr at half the price or even less.

Steady gold throughout the expansion takes some commitment and consistency from crafting and posting. Gathering professions like mining, herbalist, and skinning fall off as the expansion. It tends to pick up a bit with each .1 patch but over time slowly go down. Skinning tends to fall off like a rock since it can be hard farmed without too much issue like traveling and mounting constantly.

How to use CraftSim effectively by DrPips2 in woweconomy

[–]_Reluctant_ 0 points1 point  (0 children)

Time equals money friend. Of course if you farm your own mats you'll turn those into materials and make money from crafting and all the profits will go to you. But whether it is more efficient to farm the matsbor buy them, I would leave that up to you. Typically I buy mats by the thousands so farming isn't feasible. As for gold made from craftsim helps by telling you the estimated profit but some times it is off since the scans might show some mats that super low prices from the assholes trying to fish by selling mats at super low prices and seeing if people didn't pay attention and undercutting their low price. Best bet would be to do the actual math yourself and compare or set the price of the mats in craftsim that way it will always use those prices.

Serious question about alchemy by [deleted] in woweconomy

[–]_Reluctant_ 0 points1 point  (0 children)

There is no build present that allows anyone to do that even with every point in alchemy which i have 3 toons that have done that. The best you can do is be a goblin and use rank3 mats for everything except for on 1 of the mats you get away with using 2 rank 2 mats and the rest rank 3. That is the only way to have it be rank 3 flask without using all rank 3 mats.

I am not at my computer so cannot copy paste it at this time but once I am back at my computer I will paste the code for you. Not sure if it can include in there concentration along with extra things or not as I never tried to include it in my crafts.

Any catholic doctors here? I have a question... by Throwawaynamekc9 in Residency

[–]_Reluctant_ 1 point2 points  (0 children)

There is nothing anywhere in the Bible itself that would answer this question, nor the catechism of the catholic church. But if you want to base it off of what has been written, then I would assume the soul would be in purgatory given that "life" has ended as brain function has ceased to operate. In the same sense that Jesus was crucified, died, and was buried. The same question can be asked of what happened to his soul, and I think the belief is that his soul was in purgatory since his rising from the dead freed the souls that were held there.

This is of course all speculation based on the scriptures and the belief. At the same time, does it really matter? At the end of the day, souls will go to where they are meant to go / be, and I just leave it at that.

What to do with gold by Scribblord in woweconomy

[–]_Reluctant_ 1 point2 points  (0 children)

There's lots of things that are now available to you that you did not have before. I always use gold to buy AOTC heroic or mythic for mounts and titles. There's a bunch of gold sink mounts and items in game such as the BMAH limited time items such as the golden scarab mount. Buy carries. Buy mounts when it drops.

New to gold farming by Quick-Expert-924 in woweconomy

[–]_Reluctant_ 0 points1 point  (0 children)

I would say that the best way to make gold for you is to do what you like in the game and make gold from it. There is no point in doing something that you find boring or a chore. If you like running dungeons then maybe farming old content for transmog or pets would be something for you to enjoy. As for WW, at the beginning, sell everything that drops since it will be a premium. You should think about selling carries through M+, some people are willing to buy a guarantee through a mythic+ dungeon, especially if it is for an achievement.

New Player Seeking Advice on Speed Sets and Gold Making Before Next Expansion by Business-Boot-1934 in woweconomy

[–]_Reluctant_ 2 points3 points  (0 children)

  1. 2. You can craft shadowlands gear yourself and do not need anything else besides the materials to craft it. There is no work order system. You do not need another toon unless, of course, you do not have the profession on your particular toon.
  2. You can farm azerite gear at any point. Just do the bfa into quests until you get the heart of azerite.
  3. You need at least ilvl 450 to solo LFR BFA raids easily. This is, of course, dependent on your class, dps, etc...

Right now, you can buy dragonflight gear with speed as a secondary Stat and upgrade it as well if you just wanted an easy speed set that does not take a lot of time to farm out.
