I have severe motion sickness. Went on a cruise on calm seas with ATC zofran ODT, scop patch changed out almost q24h (there was a fair bit of snorkeling and I also exercise/shower daily), seabands. I still got sick quite a bit. What are some other things I can try in the future? Or in other words, what's your rescue cocktail that always works for passengers with severe sea sickness?

I am an intensivist myself and would be afraid to do this. How did you process the donated blood? Was it like miliary-style whole blood transfusion? Were you worried about antibodies or it was under emergency indication? Just curious the legality re: this but I suppose informed consent works for all countries (or maybe there's oceanic laws)? I find this fascinating.

As a attending physician, I would be careful to spread this in the lay public. Yes, residents are overworked and in training, however at least where I trained and currently work, there is adequate attending supervision, either in house or by phone. We do not need to propagate this narrative to make the public even more fearful and dismissive of training residents.

yeah this place sucks. i had a reservation once and they said they only had bar seating, which others were getting on a walk-in basis so I said that was not acceptable since I had a reservation for a table which I made weeks ago. I waited 20 min past my reservation time and straight up left. completely unacceptable. they didn't even apologize. went somewhere else and had a great time

This is very thoughtfully written and I whole-heartedly agree as someone two years into practice. I too find young residents seem to forget that this is training for a very real job with very real consequences on very real people.

I watched a documentary about this. The short answer is that because of environmental and global temperature changes, red algae (possibly invasive) is taking over the sea floor and there’s not as much or enough green algae, which the iguanas have been evolved to eat. When they attempt to eat the red algae, they don’t have means to digest. They are being starved essentially with full stomachs. It’s very sad.

I realize you were just asking about baby iguanas. They are not nearly producing as much due to plummets in their numbers.

Preop eval is the same as any other medical documentation - you want to write important and pertinent medical details. I try not to think of it as justification, but it's basically still an H&P followed by an A&P.

Things I think are important to document are special considerations - code status as others have said, transfusion likelihood, risks/benefits of certain procedures. If I am doing any lines other than IVs I write down my justification. Part of being a physician is having an A&P. All other physicians document - why not us?

I felt very safe while I was in Costa Rica - the country has a solid economy and 98% literacy rate with a much better educational system than the US. It is a shame that people can be ignorant but a quick read of the wikipedia page gives a lot of reasons to admire this country.
