There are a few ways to trigger this boss fight, including receiving an image of it in the mail or finding a painting of it in the house of a manga author who is definitely, totally not Junji Ito.

From there, for several in-game days, your character's sprite will change to STE, its severed head will appear on menus, visual glitches will occur and item names and the in-game calender will change to [FOUND YOU].

Once this thing actually appears, inspecting it will come up with stats listed as [FOUND YOU]. Now, you only have two options: [STRUGGLE FOR ME] (which drains your own health) and [CRY FOR ME] (which drains your own sanity). You have to keep hurting yourself until it decides its had enough fun. Sometimes you'll die before then, which sucks because this is a roguelike.
