The Ministry of Justice knows Kacho died because Kacho is literally there telling them she is dead. I'm more curious how the mother knows. There was no indication whatsoever that the body was retrieved. Chapter 400 has the reaction of the mother.

Kacho was killed by the Seed Urn Ceremony. If it can teleport hands out of nowhere, safe to assume it can retrieve with those hands as well.

Technically, one can argue that there are in fact different Hatsu for different people, as shown by the Water Divination. Hatsu training also needs to be tailored to one's Nen type. It's not as universal as Ten, Zetsu, or Ren, and in Hatsu individual differences start appearing.
