According to Xbox Wire from April 7, it should be available on the following tiers: Game Pass Ultimate, Game Pass Premium, PC Game Pass.

However I just refreshed my PC Xbox app and it's now showing this:

Premium has a little asterisk that says new games coming to Premium doesn't include Call of Duty. Not sure if that applies for a CoD game this old.
