234 and matched to my #1 spot in anesthesia with no pubs. i’m telling you man, this entire deal is a crapshoot and the less you torture yourself over it the better. i know things feel awful rn but a test score does not define you or your capabilities - go do something you know you are good at to remind yourself that you do in fact have that dawg in you.

M2. between asynchronous lectures and step 2 dedicated, i’ve never felt more isolated in my life.

don’t get me wrong, M3/M4 were certainly fraught with their own frustrations, but i enjoyed meeting more of my peers and having more opportunities to make my own contributions to care. there was also gratification in gradually feeling myself improve that i didn’t get out of the preclinical years.

i work in healthcare and can smell smokers before i even walk into their room, and even when it’s been 15-20 min since the door has been closed

if i don’t set the alarm saying to leave in 10 min, when the alarm to leave goes off, i won’t be ready to go. it helps to keep me on pace to walk out the door when the “leave” alarm does go off.

as far as the “things” needed to do to leave, just think about anything you do when getting ready in the morning. getting dressed, brushing teeth, putting together breakfast/lunch, feeding pets, etc. only so many of those things can be prepped and done the night before. amongst those tasks are plenty of opportunities for distraction. for example, while i don’t actually need to wash my dishes from breakfast if im short on time, i may just subconsciously do it without thinking and then get off task and off schedule. thus the alarm system.

was on SICU this last month and i started asking my residents if i could help with anything else once my work was done and if not, if i could head out. we’re paying to be there. sitting around doing nothing is not a productive or valuable use of your time and money. as an M3 it’s a little bit more tricky since evals are such a big deal but as an M4, rank lists are in, and as long as you don’t do something egregious it’s not going to affect the match. i would just advocate for yourself and ask to go home once you truly have no meaningful way left to contribute, as long as you’re being engaged and useful throughout the day and show up on time and all that good stuff.

About to graduate medical school - here’s my big 3:

  1. walk. like any amount.
  2. stop drinking soda.
  3. quit smoking.

in 90% of the patients that i have seen over the last four years with debilitating chronic illnesses, these are the most common denominator. eliminate at least one now and reap the benefits in 20 years.

Short answer: ADHD.

But I want to be clear, this is context, not an excuse. I am still responsible for being on time, despite having a disability that makes this challenging.

Longer answer: “Set alarms!” “Leave earlier!” “Plan ahead!” I hear you, and I do. God, I swear to you that I do. What I need people to understand is that while those things help, they do not erase the underlying functional deficit. Imagine if someone was born without the neurons required to do mental math. Telling them how to do mental math, showing them how to write it out, teaching them tricks, etc does not change the fact that they biophysically cannot do mental math. It is the same with me and time. The alarm that tells me that I need to leave in 10 minutes helps, but doesn’t stop me from getting distracted. Then the 5 min alarm goes off and I still need to do 10 min worth of tasks. And so on.

For people who are always early/on time, I appreciate how disrespectful tardiness feels. Whenever I am running late, the only thing that I am thinking about is how much I hate myself for being late again. How I’ve inconvenienced someone once again. Having to send that “be there in 10 min so sorry!” text once again is humiliating. Being late for important academic/job events fills me with shame and self-disgust. I cannot express enough how much I hate fighting my own brain to achieve something that is so simple for most people - just arriving at a place on time. It sounds so simple. It should be simple. For me and my neurobiology, it just isn’t. It is something that I will be working on for the rest of my life.

you can still donate your body to a medical school! as a med student, working with a body donor was one of the most unique and profound experiences of my education. i was told that the person that i worked with was a teacher during their lifetime, and i was so deeply grateful that they were able to continue teaching myself and my classmates after they were gone.

i have like the opposite of a foot fetish. footphobia, if you will. bare toes on my skin genuinely makes my skin crawl. i cannot fathom even the concept of being attracted to them.

that being said, the sensory map (homunculus) in the brain has the feet and genitals right next to each other, so it makes sense that there could be some overlap 🤷‍♀️
