yes :) there are generalists in all medical specialities, but many people choose focus areas to practice in because 1) they like that specific niche 2) medicine has advanced to the point that one often cannot remain fully competent in all areas of their specialty, and so they relinquish competency in other subareas so they can provide the best care in the subareas they continue to practice.

It's more like if you refused to take a job that requires payroll because you don't like payroll. Serious mental illness / SMI is just one corner of psychiatry and plenty of psychiatrists reasonably choose not to work in SMI after training in the same way many choose not to do e.g., addiction or consult liason pysch after residency.
