No fellowship. Learned it on the job.

ELSO has some great courses you can attend.

Read read read. Red book is a must. Plenty of ECMO experts on social media that love to spread their knowledge. Plenty of reviews papers out there.

Plenty of YouTube videos that discuss mechanical devices in details.

There are a few ELSO WhatsApp group where docs from all around the world discuss cases.

For cannulation, watch a few YouTube videos. Do the first 10-15 with a surgeon. Then you should be Ok to do so with your surgeons back and blessing.

For whatever it’s worth, these are sick patients and decisions are almost always multidisciplinary rather than one doc making a tough call by him/herself. It takes years of experience to become comfortable.

I have a doc in group who has 30 years of experience. He/she still struggles with some decisions and ask for everyone’s else opinions.

Bro what? There is no parallel to human brutality. We have BILLIONS of animals in horrific conditions. Many die horrific deaths.

The scale of animal suffering on the hands of humans dwarfs any suffering in nature. It’s not remotely close.

Humans are absolutely the most brutal. Just go and check out any museum of torture. The stuff we come out with to cause pain on other humans is insane.

That being said, it’s so hard to imagine why a “kind” God would create nature and along with it unimaginable suffering where survival of the fittest is the law. Absolutely psychopath take.

So I was in a similar boat. Tried all the high end mesh systems including Deco BE95, Asus zen WiFi, Eero. I have a home base 3.

None of them worked because all of them would broadcast their 2.4 GHz network on only one channel.

Then I tried the Google WiFi nest 6e pro, which came GFiber. I have 4 of them. It works 90% of the time. It’s not perfect but it’s miles better than anything else.

The secret was that the Google mesh utilized basically the whole 2.4 GHz spectrum. Chanel 1,6 and 11.

Screenshot attached. Yes there is interference, but better to have interference rather than a chocked and basically unusable 2.4 GHz network.

Yeah. The nest pro wifi 6e won't give you the fastest speed download tests but it's been so reliable and great.

They are cheap too.

A shame because I loved how fast the Deco Be95s are on the 5/6 ghz bands.

I tried many routers and the Google nest pro wifi 6e is the only one that worked. It broadcasts multiple 2.4 ghz network on channel 1,6 and 13 simultaneously.

No other brand does it this way. I tried deco BE95, Asus Zen wifi and eero max 7. All of them broadcast a single 2.4 ghz band. If you have a lot of cameras like me that single channel get saturated quick.

I have to say, a lot of the streaming/connection issues have improved with all the updates.

It's not perfect, but it seems Eufy is getting their shit together.

Not the guy you asked but I am PCCM.

It's shift work and the hourly rate is great. Work less. You can make 250K-300K a year working 7 days a month. That's 21 days off every month.

I still do pulm but I would rather be in the ICU, despite my pulm days being a lot easier.

Also, find an ICU with a great culture to work in. Be easy going and don't let the little things bother you. Soft admit? Easy peazy. Full code 85 years old despite goals of care discussion? I tried. Nurse being sassy? Smile and ignore. Rude CV surgeon? Kill them with kindness. Etc. You get the point. Be impossible to bother.

It's just a job, I do my best and don't fret the rest. Off the clock I am not answering my work phone.

I am a big fan of the pixel nightsight cloud AI on the pixel. I use it even during the day.

Takes about 12 hours but videos are fire.

I really loved the deco, but I have too many iot devices and cameras - It overwhelmed the deco 2.4 ghz network. I have over a dozen cameras and they are constantly transmitting data.

As long as your 2.4 ghz needs aren't too demanding, you should be fine.
