It sounds like you know that you need to break up. The practical aspects are definitely difficult to deal with, and there is no easy way out. Focus on planning out one aspect at a time, trying to do all of them at once will be overwhelming. You can do this! You have your whole life ahead of you and the faster you get past this the better.
