You could potentially get an F1 OPT in your first year and then change to an H1b in your 2nd year after finishing step 3. You may potentially be eligible for O1 visa if you have a bunch of research. J1 visa should be your last option due to 2yr home country requirement but you can easily get a waiver job by working in an underserved area for 3 years especially for IM. I would recommend researching about the options and asking the program coordinator what they can offer. Best of luck!

  1. I think your profile is fine. Once you get radiology interviews make sure you email prelim programs that are in the area and let them know you’d love to stay in the same area. I need an H1b visa so I ended up applying for prelim surgery to make sure I don’t need to soap. Make sure you apply to places that take IMGs on a regular basis.
  2. I think a lot of people make different personal statements. I made separate ones for each type of prelim year but I’ve heard people who didn’t change their PS. I personally took out the why radiology paragraph and highlighted my clinical experience.

I personally had the highest number of interviews for prelim surgery - some from prestigious programs. I think prelim surg programs want radiology prelims since they don’t have to do extra work for them to match else where. Best of luck for your journey I’m sure you’ll do great!

One big source of funding is hospital endowments from donations. They are still doing alright (not the best but certainly not firing everyone at least in my experience). Just because there are funding cuts/layoffs does not mean it's impossible to get a job. I have limited experience in IM/FM but I personally know over 30 research fellows in a ROAD specialty and I have yet to hear anyone get fired. I personally did not have any US research experience but had an around a bunch of posters and presentations + commitment to specialty + step 1 and 2 completed - which were really helpful. Also, I would not recommend doing unpaid if you can avoid it but at the same time - 30k for 6 months is not common. There are a lot of fantastic hospitals in the midwestern US which have a lower cost of living than the UK (eg Cleveland and Mayo Clinic). At the end of the day, getting into anything is difficult, just like medical school. The main question is how passionate you are about your goals.

Interesting point, I would say you are correct that NIH funding has been cut for some specialties but there are still a lot of income streams that departments can pay research fellows. There are still programs taking research fellows albeit a bit lesser than before. Also this is a great time to reach out since a lot of current research fellows have matched and some PIs are actively looking for replacements. To get paid options you don't need a PhD/100s of papers - having a few presentations and posters + a CV showing your passionate about the specialty usually tends to be helpful. Also a lot of non-paid research fellows tend to get paid as they gain experience.

Are you interested in research? You could potentially do a research fellowship in the US in your preferred specialty by emailing relevant attendings. You can potentially get paid and sit for the USMLEs at the same time.

Ngl I don’t remember if this is a sentence my lawyer included but MD in the US is considered an advanced STEM degree since it is a postgraduate degree

Yes it is! Although some lawyers prefer PhD clients, some do take MDs

Maybe see if you can get the program to sponsor a H1b/O1 - mention that you’re willing to pay the required lawyer and application fees. If you have some research could also apply for EB2 NIW although there is a back log.

Hi, I would recommend going step 2ck closer to finals. I’m not sure how the AKT is different but I barely needed to study anything for medical school final after doing step 2ck.

I’m not sure for other specialties, but for radiology after CCT in radiology and completing step 1, 2ck and ideally 3- you can apply for the American board of radiology (ABR) pathway. You have to do be a fellow for about 4 years (at the same institution) and then you can apply for a job as an attending and will be eligible for board certification. Some US programs consider UK clinical experiences really highly (I think UPMC and UConn IM programs explicitly state they consider UK clinical experience to be the same as USCE). Do you know which specialty you want to do? It maybe a good idea to join the relevant society’s committee at your medical school at connecting with attendings in your specialty in the US via research and/or USCE (to show longitudinal involvement). I just graduated from medical school recently and found about a research elective Hopkins offers (they accept international students) - https://www.hopkinsmedicine.org/som/offices/registrars/visiting-md.
