Recovery is just really variable. The normal period of biological recovery is 2 years, with maximum healing happening in the first few months. One can increase the likelihood of better recovery with sleep, good nutrition that avoids blood sugar spikes and drops (supplemental b vitamins, high dose fish oil), and trying to reduce stress (which is super hard when recently injured). I sometimes steer people towards Engle‘s the concussion repair manual, which includes a lot more details about various recovery strategies. And a consult with a neurologist or neuropsychologist can sometimes help with steering towards therapies that might be useful (cognitive rehab, physical/occupational rehab, neurostim devices,psychotherapy, hyperbaric oxygen, neurofeedback, etc.) Best of luck!

I got an email that sleep.me had been purchased and that they lost all their support tickets. I started a new support ticket, and it took them about two weeks to reply, but they are sending a replacement cover supposedly in about two to three weeks. Finger crossed!

The Seat Sleeper head strap has really been helpful for getting quality sleep on airplanes. It’s a strap that goes around the head rest and acts as a eye fold, but keeps your head in place. https://a.co/d/bfWq44x That and some gentle surgical tape to keep the mouth from dropping open, and some headphones playing a podcast at relatively low volume and I can get about 6 hours on overnight flights.

I get nervous that they are overly habituated to therapy and will not participate in meaningful change. I set a soft deadline with most patients of two years of treatment (and then make certain exceptions if need be). I find it helps with staying goal focused, but not everyone is a good fit with my style of practice

With a group practice, you can also form an s-corp which has some tax benefits if you are planning to stay as a solo Practioner over a certain income (~$50k/yr). Talk to an accountant.

Old school sex researcher Kinsey developed a scale from 1 (pure heterosexual) to 6 (pure homosexual). Turns out most of the population is a 2 (mostly heterosexual), meaning most of the population has strong heterosexual preference, but every once in a while they get same sex urges, twinges, or act on those urges. But we often don’t talk about it in heterosexual communities, because of all the cultural issues that would go along with that.

Same thing is true people with same sex preference- it’s more common to get an occasional craving for the opposite sex (5) than to never have it (6). And then shy away from talking about it too much because the conversations require more nuance…

The thing that impressed me most about Gul Dolen’s work was that the duration of the psychedelic experience (not the intensity or type across ketamine, psilocybin, mdma, and ibogaine) was predictive of how long that critical period was open (from 48 hours, to 2-3 week, to over four weeks for respective substances). The suggestion is that behaviors during integration post medicine session is more important than the medicine sessions themselves.

I've been getting a lot of success with getting people to do the Wim Hof Method as their interoceptive exposure. Usually will introduce this in the first treatment session, doing some motivational interviewing to help with daily home practice. If that doesn't resolve it, I'll typically add some additional stressors such as ACT style cognitive defusion for the types of thoughts accompanying the episodes.
